This is where you note what you would like to talk about in the session. This might be an issue client with a client, or something about your business. Whatever you want to discuss with your supervisor, jot down the important points to ensure nothing gets missed.



This is for use in the actual session. You can write notes based on the points made on your agenda, and expand on them. Record useful tips from your supervisor or any information that will help you move forward.

Scribble pad

This is a doodle pad that can be used as a white board allowing you to draw visual diagrams, or to allow you to make a copy of anything visual you might see or need to note in the sessions.



This section is a space to allow you to reflect on the session. Use it to record anything you need to act upon and the results of your actions between sessions. For example, “Mary suggested I use the empty chair technique with Gladys, as it would allow them to look at their interpersonal relationships. I felt very confident using this technique in my session with Gladys, but would like to know more.”
