What is the relationship between Psychology and Dermatology?

Most of you might have at least asked one of the above questions at least once in your lifetime. If you belong to that category, this book will be an excellent choice for you as the author is trying to answer all these questions convincingly. She, in fact, mentioned this topic in her other book, How To Tell Your Skin To Heal Itself. This book explores this topic more deeply.

This is not a medical book where there is a plethora of medical jargon bombarded straight at the readers bolt out of the blue like seen in Medical literature. This is, in fact, a book mainly targeted at every layperson written in the language that anyone, including a teenager, can understand. However, it will also be helpful to everyone, including health professionals, as there are some misconceptions regarding skin diseases, even in those working in health care.

She will teach you the core concepts of Rational Emotive Behavior Therapy (REBT) that will help you manage emotions and alter beliefs to overcome psychological diseases

What I learned from this book
1) What are the difficulties faced by dermatologists in their daily life?
Becoming a dermatologist in a country like India is very difficult. To get a PG seat in Dermatology is extremely challenging. As Dermatology is considered a lifestyle specialty in many countries, many people opt for the same, and only the best among the Doctors can become a Dermatologist.

Dermatology and Cosmetology is the field where many advances are happening, and it is challenging to deal with a patient with skin disease due to the stigma and psychological aspects of the same.

Patients respond to treatment differently, and it is difficult to bring instant results in the case of treating some skin conditions. Treatment relapse is another vital thing that all Dermatologist has to tackle. The author discusses all these aspects of a Dermatologists daily life in this book
"Like many in my fraternity (or so I want to believe that I am not the only one), I have come across skin illnesses which have made me peel off my skin from frustration. Not to forget the incessant cases of hair fall that also have made me lose hair and a good night's sleep. There are so many cases, where despite knowing the diagnosis, I haven't been able to give them remedial cure or prescribe creams to alleviate their issues (Most patients with psoriasis and vitiligo are bobbing their heads up and down as they read this). And even if the creams did work, the diseases came back (My patients are most likely cursing me under their breath). And hold on… I am also talking about simple cases of dandruff that don't seem to go away! Ringing a bell with you now? Although I have been able to deal with skin diseases with a happy endings and forever-cured-afters… I have had my fair share of cases, whom I have let down. Even today, after 15 years of experience with skin diseases, I do have hard days when I know I could have done better."


2) What are the common misconceptions associated with your hair?
Hair is considered one of the most vital parts of the body by many. I remember seeing a patient in the Med school who was suffering from Cancer and undergoing chemotherapy. She told me that she can tolerate the Cancer but can't tolerate the hair fall associated with the chemo. That is the extent to which any skin condition can affect the patient.

But does our hair require this much respect and consideration from us? The author helps to remove some of the basic misconceptions associated with the hair. I wish all the non-fiction authors showed courage and honesty, like Dr. Pradnya, to clear our misconceptions related to these topics.
"Hair is the most useless part of the skin to be honest. There is a reason why as we evolved from our ancestor langurs we shed the hair and kept it only the head and few other parts of the body. But it wasn't for protection, but for vanity. And if you ask me, I don't know how much of a beauty accessory it is. Because even baldness seems to have a certain appeal to most individuals. Dwayne Johnson, the Fast Furious dude Vin Diesel …. Cool looking, aren't they?

Moral of the story: Hair could be important but not as important as your personality in total."


3) What is epigenetics, and how will it help us to know more about skin diseases?
There is a tendency among people to blame their genes when they find a particular endeavor difficult. This same complaint is heard from people from different walks of life, from bodybuilders to people suffering from conditions like vitiligo to even teenagers when they suffer from conditions like acne. The author shows us how to understand more about skin diseases with the help of epigenetics.
"Epigenetics is the study of how your behaviors and environments alter the way your gene works. Simply put, even though a person may have the gene for psoriasis or, vitiligo or even acne, does not mean that the person will suffer from it. It is when the gene expression is triggered due to the influence of external stimuli is when diseases are manifested. So don't go blaming your parents please!"


4) What are the four basic factors that influence our personalities?
The author mentions the four factors that are needed to develop a better personality. Even though all the four factors mentioned are not essential to maintain a balanced and productive life, if all the four are in an imbalance, it might make the person's life dysfunctional
"The four basic factors that influence our personalities are:
1. Nature – the genes we carry
2. Nurture- our upbringing as children
3. Culture- the values we are raised by
4. Environment- the physical surroundings we are exposed to"


5) How does REBT help heal the skin?
REBT stands for Rational Emotive Behavior Therapy, which was developed by Dr Albert Ellis. Dr. Pradnya is one of the few Doctors who is implementing REBT in treating patients. REBT helps manage emotions and alter beliefs to overcome psychological diseases. It can also be used successfully while treating in almost all fields of Medicine.
"All the illogical thoughts are leading to distress that release stress hormones like ACTH, Adrenaline, and Cortisol. The system called the Neuroimmuno cutaneous endocrine (NICE) is getting activated thereby causing your skin disease. Dealing with your stress will help in regulating the NICE. And REBT shows you the way."


6) What are the three basic values that REBT teaches, and what are their core logical beliefs?
The author gives a more in-depth analysis of REBT in this book
"The three basic values that REBT teaches are
A) Unconditional Others Acceptance (UOA)
B) Unconditional Self Acceptance (USA)
C) Unconditional Life Acceptance (ULA)

Core Logical Beliefs
A)UOA
- Others will treat me unfairly from time to time
- There's no reason why they must treat me fairly all the time
- Those who treat me unfairly are no more worthy and no less worthy than me or any other human being

B)USA
- I am a fallible human being; I have my positive points and my negative points.
- There is no reason why I must not have imperfections
- And despite my positive points and my negative points, I am no more worthy and no less worthy than any other human being

C) ULA
- Life doesn't always work out the way that I would like it to
- Even if life worked out the way I wanted it to, doesn't mean it will continue in the same way
- There is no reason why life must always go the way I want it to or whenever I want it to
- Life is not necessarily pleasant but it is never awful and it is nearly always bearable"


7) What is hedonism
Oxford Dictionary describes hedonism as. - ​the belief that pleasure is the most important thing in life. The author teaches us the art of self-love and responsible hedonism in this book.
"The word hedonism is derived from the Greek hedone ("pleasure"), from hedys ("sweet" or "pleasant"). And although hedonism is only about seeking pleasure and immediate gratification, responsible hedonism is about seeking pleasure keeping my and others' interest at heart. It advocates moderation in seeking pleasure. And it introduces the concept of self-interest. Which is very different from selfishness. And a lot of people confuse the two as being the same."

What could have been better?
The only negative I can point out about this book is precisely the same as what I mentioned about the author's previous book, which is I wish that this book was a longer one. But I can't complain about it here, though, as she gives us a comprehensive analysis of REBT and other topics she planned to discuss in this book. I know it is difficult for a Doctor with a busy professional life to write a 300-page non-fiction book. Still, I am glad to hear that the author is working on another book, which will be a longer and more elaborate one that will be published soon.

If you are someone who had to tackle a skin disease at least once in your lifetime, this book will be a good choice. If you are someone suffering from a chronic skin condition, you should never miss this book, as it might significantly improve how your body will respond to your Doctor's treatment. The principles mentioned by the author can also be applied to other fields of Medicine.
