Psychoanalysis is a theory developed by Sigmund Freud that aims to cure neuroses, i.e psychological disturbances that have no organic basis, e.g., phobias, depression, addictions or functional stomach upsets. These disturbances spring from unconscious strivings developed in order to cope with life despite fears, isolation, etc.
Karen Horney believed - contrary to Freud - that it is possible to perform a psychoanalysis by the patient himself alone. Therefore, she wrote this book as a manual for layman patients so that they could get rid of their neurotic symptoms as well as make them free for the development of their best potentialities.
Book summary
Neurotic trends (NT) are unconscious urges that develop early in life (combination of temperament and environment). They give the person feeling that he must stuck to the established path at all odds. They must not be confused with character traits however: for instance, neurotic perfection differs from just “being more orderly” in the fact that the former is pursued compulsively (with disregard for self-interest) and the sufferer gets frustrated when these compulsive pursuits are ineffective.
Course of analysis
For each NT (a person can have any number of NTs):
(1) Recognition of NT
(2) Discovery of its causes, manifestations and consequences
(3) Discovery of interrelations with other parts of the personality, especially with other neurotic trends
The actual analysis is a long-term process (can take years) consisting of two main techniques, namely: (A) regular free-association (FA) sessions, and (B) systematic analyses of FAs’ outputs as well as interpreting one’s dreams.
Over time first appear the least repressed NTs (so usually these are not the strongest with the greatest influence on personality). It also happens that the patient later discovers that some of these were not actual NTs but only symptoms of secondary defenses.
Free association
During FA, he expresses himself as completely and frankly as possible, i.e. without reserve he says all that comes to his mind regardless of whether it appears trivial, off the point, irrational, embarrassing, etc. He talks about his ideas, memories, thoughts about self and others, beliefs, wishes, fantasies…
- In self-analysis instead of talking to a psychologist the patient writes down notes or catchwords
- FA lacks directions (no reasoning allowed); it must be spontaneous
- It’s important to take notice when tempted to conceal something
-The goal is to enable to understand how the patient’s mind works, to reveal the structure of personality by recognizing factors that were unconscious
Analysis
- Attentive analysis of the notes taken during FA
- Goal lies in finding unconscious forces, they must not be selected prematurely however
- Look for details, e.g. if patient every session emphasizes his superior IQ, then it has paramount emotional value to him
- Look for repetitive themes, contradictions, exaggerations, reactions of shame
- Discover the points at which he turns from cooperation to defense, identify themes the patient omits or deserts quickly
- When we find an observation or gain an insight, it must be examined how that manifests in various areas, what consequences it has, which factors of personality account fo it
Example: A person is tongue-tied when he wants to participate in discussion because he is afraid of possible criticism. If he allows this observation to take root within himself, he will see that the fear is out of proportion to any real danger - he will wonder why the fear is so great that it prevents him from speaking and even thinking properly. He will try to find similar difficulties in other areas of his life and what forms they take. Basically he will perform steps (2) and (3) from the “Course of analysis” paragraph.
Resistances
Analysis invokes two groups of opposite forces - the interest of the one group is to maintain unchanged the illusions and safety afforded by the neurotic structure; the other is to get freedom and strength by overthrowing the neurotic structure.
Recognizing Resistance Forces (RF)
RFs are set in motion when patient’s security claims are jeopardized, illusions questioned, etc. Person suffering from RFs is like a person frightened by thunderstorms, who is not only terrified by thunders and lighting but even by a cloud on far horizon. The sources of resistances are the sum total of a person’s interests in maintaining the status quo. When we approach any domain that is tabu for the patient (repressed) he will react with anger or fear and set going defensive action. (Patient with strong defense mech. will react emotionally when anything is questioned).
- Resolving a blockage is number-one priority
- It helps to review notes that precede blockage to find the point of departure
- Try to associate to a resistance, i.e. consider the particular manifestation of the blockage and let thoughts run freely
Basic types of resistances
(1) Open fight against the provoking problem
(2) Defensive emotional reaction
(3) Defensive or evasive maneuvers (Evasive maneuvers in self-analysis may take various forms and may develop at any point, e.g. may prevent a person from starting to analyse a problem, may impair the value of his FA, invalidate findings… So he should be very wary about the times when he feels distressed/fatigued/irritated when he should do self-analysis)
Analysis of dreams
- Dreams are one of several important sources of information, they however don’t give a static picture of feelings of opinions but are primarily an expression of tendencies
- They may reveal our true feelings
- They are governed by wishful thinking, i.e., give voice to our strivings and often represent attempts at a solution of conflicts bothering us
- To understand a dream we must connect it with the actual provocation.

- Anxiety often appears when there are two conflicting feelings toward one subject, e.g. when we both resent someone and crave his admiration - then, it’s necessary to suppress one while acting with the objective of fulfilling the other
- In all neuroses self-confidence is impaired and fictitious notions of superiority substitute for it
