Basic Plot: What should a doctor do when a strange man comes in off the street with an even stranger tale to tell?

This one started off creepy, then kept piling on the layers until a normal reader would begin to wonder how on earth the author could top the whole thing off. Then he did. The characters are definitely secondary to the crazy events of the story, but it doesn't detract from the overall effect. And boy does it have an effect.
