Physiological interpretation of the cardiotocograph (CTG) involves careful scrutiny of the features on the CTG trace to determine the oxygenation of the fetal central organs, and efficiency of utero-placental oxygenation as well as incorporation the wider clinical context. The implementation of Physiological Interpretation of CTG Guidelines produced by 44 CTG Experts from 14 countries involves excluding a Chronic Hypoxia and Pre-existing fetal compromise ( Is THIS fetus FIT to undergo the Journey of Labour with progressively worsening hypoxic stress? ), and then asking the question “How is THIS fetus”?, each time the CTG trace is analysed during labour.
Physiological CTG Masterclasses were commenced to disseminate and democratise the knowledge of fetal pathophysiology to improve maternal and perinatal outcomes in 2006, following the unfortunate hypoxic ischaemic encephalopathy (HIE) sustained by 13 neonates as a result of lack of appropriate training on CTG interpretation at a Teaching Hospital in London.
The author has conducted over 150 one-day and 2-day Physiological CTG Masterclasses in over twenty countries , including Italy, France, Spain, Lithuania, Estonia, Denmark, Netherlands, Faroe Islands, Iceland, Ireland, Sri Lanka, Singapore, Pakistan, China, Greece, Germany, Hongkong, Jersey, Oman, Switzerland, Philippines, the UAE, Finland and Australia, in addition to several centres in the UK, from 2006 to 2020. Following these Physiological CTG Masterclasses, several hospitals stopped using traditional “pattern based” CTG guidelines and their “normal, suspicious and pathological” classification tools, and abandoned the use of FBS . Moreover, after implementing the principles of physiological interpretation of CTG traces in their daily clinical practice, many hospitals have demonstrated > 50 % reduction in the rate of hypoxic ischaemic encephalopathy, and some have received National Awards for improvement in perinatal outcomes.

This Course Manual for Physiological CTG Masterclasses highlights the principles of physiological interpretation of CTG and it is intended to be a companion to those attending, or intending to attend a Physiological CTG Masterclass. It will be also a useful revision-guide to those who have attended a CTG Masterclass.
