I wrote the book at the behest of my mother, because I'd just finished the final publication of The Golden Ticket: Stories of Love, Madness and Other Afflictions, and it was my birthday, and we were standing in the kitchen, and I asked her, "What should I wrote about next?"

And she thought for a moment, and then a little ghost of a smile appeared on her face, and she said, "Smoking."
