Yes, Agree, We Can Do It... Then Uncovering the Disconnect in Team Commitments
NASA, an icon of innovation and daring missions, faced a tragic reminder of what's at stake without a culture of open communication. The loss of Kalpana Chawla and her 6 other fellow crew members aboard the Space Shuttle Columbia teaches us a critical lesson not just for space exploration, but for every time we come together as a team to create value for customers. Ever experienced a project planning or product refinement meeting where there were plenty of YESes but not matched by the quality of outcomes? Ever puzzled over why projects derail and deliveries are dragged despite apparent consensus?
This book dives deep into the disconnect between the collective agreement in meetings and the reality of execution, exposing the silent productivity the absence of psychological safety. It explores the reasons team members, despite seeming agreement, often refrain from voicing concerns, asking pivotal questions, or expressing doubts, leading to unanticipated hurdles in service or product delivery.
But this narrative isn't just about identifying problems. It's a roadmap for leaders and teams committed to cultivating an environment where open communication isn't just encouraged—it's expected.
Suitable for teams of all sizes, from startups to multinationals, this guide will likely help you bridge the gap between planning and action, transforming silent nods into meaningful engagement and repeatable results. Though the immediate consequences of lacking psychological safety might not mirror the scale of the Columbia Disaster, the prolonged impact on organizational outcomes and individual well-being can be just as profound.
Join me in this journey towards fostering a culture where every voice matters, and every 'YES' counts towards collective success.
"Psychological safety isn't confined to just one domain of your life; it's an ecosystem that affects your personal relationships, professional collaborations, and social interactions."
Psychological safety is a shared belief within a team that members can express their concerns, ask questions, and provide feedback without fear of being ridiculed or facing negative consequences.
While working in a team, imagine there is an absence of psychological safety. With illustrations, real-time examples, and analysis, the author has explained this concept in detail. In the end, there are also three case studies. A lot of inputs can be taken while working in a professional setup, especially if you're into leadership roles.
The topic might seem simple, but as you go through the chapters, you will realize the intensity of each of the sub-topics. To be honest, I took time to grasp a lot of information, though the pages are less. Overall, it was a very insightful read to me.
Silent Nods, Lost Dollars by Radhakrishnan Selvaraj highlights the critical role of psychological safety in fostering innovation, collaboration, and long-term organizational success. The book delves into how a lack of psychological safety—where employees hesitate to express ideas, ask questions, or provide feedback—can stifle creativity, hinder growth, and lead to financial losses.
Why Psychological Safety Matters
Have you ever been in a room full of creative minds but held back from sharing an idea due to self-doubt or fear of judgment? This hesitation stems from a lack of psychological safety—a workplace culture where individuals feel valued, heard, and empowered to contribute without fear of criticism.
Selvaraj explores how the absence of such an environment leads to employees merely surviving rather than thriving, causing stagnation and eroding a company’s foundational strength. Without open communication, innovation declines, and businesses miss out on valuable opportunities for growth.
The Power of Psychological Safety
The book emphasizes the compounding effect of a psychologically safe workplace, where even small positive changes can lead to significant long-term benefits. Implementing psychological safety encourages employees to address the MUM effect (the tendency to withhold negative feedback), leading to early detection of workplace issues, improved mental health, and a more engaged workforce.
Key Insights from the Book 1. The Five Stages of Team Development – Understanding how teams evolve and the role psychological safety plays at each stage. 2. Strategies for Implementation – Practical methods for fostering open communication, accountability, active listening, and constructive feedback. 3. Challenges and Pitfalls – Factors that hinder psychological safety based on different organizational structures and how to overcome them. 4. What Not to Do – Common mistakes that leaders and managers make when attempting to create a psychologically safe workplace.
Final Thoughts

In his book “Silent Nods, Lost Dollars”, Radhakrishnan Selvaraj pays attention to unattended fears in teams or the lack thereof and how these can have serious repercussions within a single organisation or the world as it relates to the Columbia catastrophe. Selvaraj uses the Columbia space shuttle incidence in the Columbia Shuttle failure case study to explain why psychological safety is also crucial in achieving successful deliverables as he does.
There are many issues that the book addresses, among which is the fact that many people accept certain tasks and fail to execute them or do them effectively, this leads to many frustrations because numerous worries and even questions do go unexpressed. Selvaraj’s explanation that this silence brings project doom is the reason why organizations do not achieve their objectives and even individuals are not healthy.
One of these important works discusses how leaders and teams can communicate better internally, avoid speaking for speaking's sake and offer more engaging feedback in the end. Conclusions that can be drawn from this are:
Capture attention and psychological safety: Creating Teamwork - valuing, listening to others and welcoming concerns.
Foster an environment of healthy debates: Stimulation of dissent, honest and open communication.
Note accountability in leadership roles: Being and demonstrating kindness and integrity

"Silent Nods, Lost Dollars". Trust me, this book has the potential to activate your brain's neurons and provoke deep thought about this important topic.
Psychological Safety at the workplace is a critical topic that is often overlooked or eclipsed by the leaders in the tsunami of other quantifiable stuff; however, when we introspect deeper, we can realise that the inception of all the productivity drivers is our psyche on that particular day. In this book, RK takes us through self-questioning, drawing on close-to-live case studies that will reignite our awareness of the people around us and help us identify the constant issues that have been unnoticed.
The short, bite-sized chapters allow readers to consume the material in one go, ending with thought-provoking, self-reflective questions that prompt introspection about their professional journeys. Readers can reflect on moments where they, as leaders, may have overlooked these issues or, as subordinates, behaved in certain ways.
What blocks people from communicating or expressing their thoughts, opinions and concerns at the workplace? This is the question that is discussed across this 150-page book. Fear of being judged or ridiculed is one dormant yet powerful force that is activated in us in situations where the brain is filled with thoughts, but the signal to the mouth is disconnected primarily due to this FEAR. I always view FEAR as FALSE EVIDENCE APPEARING REAL – it looks conclusive based on our preconceived notions; however, it is always a mirage which has the huge potential to stop our growth.
It truly rings a bell in me to pause and deeply examine the people around me in conversations.

This book came as an Author signed copy and it was timely for me to read. A topic which is most relevant for today, "Psychological Safety". Radhakrishnan (RK) talks about real world scenarios like the Space Shuttle Columbia crash, GE under Jack Welch, Therac-25 radiation therapy machine incidents where teams that could have avoided the mishaps kept quiet for fear of reprimand or the culture of the teams/companies. As someone who have worked under different types of managers and different organisations in my 18+ yrs of career, I can't emphasise enough a training and keeping a tab on the psychological safety of the work place. RK has written with active exercises to test our PSQ (Psychological Safety Quotient), delivered the key concepts with infographics and diagrams and have given case studies that simulate work environment dilemmas in this context. I would recommend this book as a read for all management executives, managers and employees as well as entrepreneurs to build psychologically safe workplaces where people thrive.
If you think you have figured out your life fully, then may be this book is not for you. Plus it is an illusion to think so. Because life is unpredictable and Psychological safety is a challenge. From his own life, author understood its true impact too. This book reflects his journey, sharing insights from his experiences and observations of others navigating their own personal and professional challenges.
For example, NASA learned a hard lesson about teamwork and open communication after the Space Shuttle Columbia disaster, which took the lives of Kalpana Chawla and six crew members. It explores why teams sometimes agree in meetings but don’t see good results later. It shows how lack of safety to speak up can cause problems in delivering projects. This guide helps leaders and teams create a safe space for open communication. Loads of illustrations and point wise explainable helps us understand it. If you are interested to read more in this arena, go for it.
This book is quick easy read, written under 150 pages and in simple English. RK doesn't try to come off as an English expert adding all those complicated words that run above my head jumbled, his approach is straightforward avoiding all these heavy words, making it easy for readers like me who just started reading(also with a lesser attention span 😅).
The analogies RK used to describe Psychological Safety are so well thought that even someone like me understood it in one read. Wow.
There's a lot of repetition in the book which I think was intentional, reading it again and again but in a different setting helped me understand the concept even better, but sometimes it got too repetitive.(Or maybe that's just my attention span talking, man I got to delete that Instagram)
And the book also has an activity part in every section, and at the end, an activity section for the overall book as well, just wanted to add it here for anyone who might need that.
