In The Great Healthcare Disruption, Dr. Marschall Runge draws on his experience running Michigan Medicine to walk readers through dynamics shaping American healthcare. He offers an insider view of what’s coming and why it matters.
The book covers a lot of ground, including retail medicine, behavioral health, obesity drugs, gene therapies, crisis preparedness, and AI. Runge shows how disruption can improve access and efficiency, and also how it creates new risks.
This book reminded me of the early push for healthcare modernization in the late 2000s, when Obama compared paper medical records to bows and arrows. The book carries that same urgency about the cost of clinging to outdated models.
One section focuses on retail pharmacies, where long waits, shortages, and limited pharmacist interactions frustrate people. New entrants like Amazon Pharmacy and Mark Cuban’s Cost Plus Drugs let you compare costs and oftentimes pay less, with the added convenience of delivery. But, when patients fill prescriptions through multiple channels, coordination gets harder and the risk of medication interactions increases.
On behavioral health, Runge talks about how underbuilt the system is. Younger people are seeking care more openly, but capacity has not kept pace and reimbursement remains low. Telehealth can help some people get support faster, but it can’t replace what you need in a true mental health emergency.
On obesity drugs, Runge argues these medications are not vanity products but so much more. They have positive impacts on diabetes and cardiovascular disease, for example. His view is that access should be improved through federal involvement.
Runge is optimistic about AI, as long as it has guardrails. He doesn’t think it’s a replacement for clinicians but a tool to assist them. For example, AI scribes can reduce the documentation burden. But hallucinations mean that clinicians can’t be hands off. In other areas of medicine, machine learning helps determine ideal ICU discharge timing, and predictive analytics flags sepsis risk early.

I won this book in a goodreads drawing.
An earnest book about how to take the disrupting technologies emerging and apply them to making the health care system in the US better. I think the author makes some good points.
However, he states 4 goals for his ideal health care system, and I'm not sure they are the goals of those in charge of it. The same people who are charge of California's light rail would not seem to care at all about access or affordability in health care, either.

Just like I’m wildly impressed that Dr. Runge somehow manages to be the executive vice president for Medical Affairs at the University of Michigan, dean of the Medical School, **and** CEO of Michigan Medicine, I’m very impressed with this book.
Appreciated the wide yet focused snapshot of where healthcare is right now and where it is likely going next. While I was familiar with many of these topics, I felt I still learned something in each chapter. Some are lighter than others, reflecting a comparative lack of definitive knowledge in that given area.
As a medical student, I suspect that if we raise PCPs to the level of gatekeeper that this book recommends we would have to dramatically cut down on patient census numbers - PCPs I rotated with were absolutely inundated on EMRs with tasks they can’t directly bill for. The idea of PCPs being aware of all care a patient seeks is an interesting one - but I question the legal liability and mental workload this places on an already overloaded medical segment.
I also very much appreciate the fact that it feels like there is little to no filler content in this book. Every fact and figure is meaningful.
Excited to read his medical thriller next!

Unexpectedly, I enjoyed the book. I had assumed it would be incredibly complicated and hard to follow, but it is actually very approachable and simple to read. In order to show off his knowledge, the author breaks down how the healthcare business is being drastically changed by technology, artificial intelligence, and even massive companies like Amazon and Google.
What I appreciated most is that he doesn’t just highlight the exciting stuff. He also talks about the real challenges, like access, rising costs, and how all these changes are putting pressure on existing systems. It definitely got me thinking about what’s ahead in medicine and how it might impact us as patients. I’d definitely recommend it to anyone curious about where healthcare is headed, even if you're not from a medical background.

A good balance of what is and what could be in a very complicated and very expensive core business of "health care", but also touches into the ingrained web of fringe actors of government, manufacturers and consumers. I don't recall mention of the legal hands muddying these waters.
Well laid out with maybe a bit too much starry eyed hopes of getting this monster under control. It would be nice to see these ideas come to fruition. However, looking at current and past 'civilizations' puts the balance of trajectory squarely in favor of the inevitable greed, control and power struggles that will tear solid ideas into their most profitable parts without a care of the carnage left behind.

New technologies are gaining ground in our daily lives and facilitating many of the tasks we perform. Medicine is not exempt from this progress and fortunately there is a lot of innovation in this field.
Clearly, when we get into a health care book, we know that we are going to find complex terms, however, it is imperative for me to clarify that the author did an incredible job making the narrative more accessible and therefore, more interesting for any type of reader.
I really enjoyed understanding the changes that healthcare is going through and, with experiences and practical examples, discovering what changes are necessary for better care taking into account the advantages and disadvantages of innovation.

To be fair, I wish I could give 4-1/2 stars. There is ALOT in this book and I could see how some would give less stars.
Dr. Runge is correct, “We can’t always get what we want”.
We need to be responsible for our own health, first and foremost. There is no miracle pill except you knowing your only body.
There is so much that needs to be corrected in our medical system in the US but it can’t happen overnight. As consumers/patients, we need to speak up if we are unhappy with our physicians and/or insurance.
If you are wondering about changes in our medical system, read this book. It will give you a different view on many things.
I appreciate the time and research put in this book.

Impressive
I read this book not only out of curiosity, but also because I wanted to understand how we are going into the future in regards to medicine. Well, by reading it, I have realized that there are brilliant heads that are bringing up many innovative projects and tools that will help humanity to take a bigger step forward in health issues. The presence of AI is crazy, it is something I had some distance from and I am getting closer and closer because it is impressive the amount of things it can do in a utilitarian way. It is really a journey through many aspects of medicine, not only impressive in a positive sense, but it also shows many shortcomings and the work we still have to do to make everything work in the best way.

A Doctor of Public Health, I know firsthand that healthcare for people with disabilities is poor. I heard international stories of children with autism who are forced to boarding school with no future. Medicaid is a poverty threshold that keeps them in poverty just by having a disability. To be honest I have a disability too, but my favorite part was meeting the Medicaid eligibility and got rejected due to income limits. In research studies there is biases against recruiting severely disabled individuals who will most likely use the medication. My question is why are researchers excluded patients who can benefit or harm with the clinical trial on a new medication, suggesting drug approval is more important than patient safety.

I have always been very interested in the subject of the medical industry; I think it is incredible how day by day they manage to improve the quality of life of people with the different emerging technologies that many times we are unaware of. In this book I understood a little more where this industry is going and I found it incredible to see how new tools like amazon or google have managed to drastically change the user experience of the clinics. It is a book with a lot of information that will bring you closer to this world and will help you understand where it is and where it is going with all the advances that day by day are found in drugs, treatments and technologies itself.

#Goodreads giveaway after I ended up winning this book in a giveaway, I was wondering why I had decided to even try for it. But after a few months, I decided to tackle it. I am actually a retired teacher who is not on Medicare yet, but did enjoy this read.
I believe Dr Runge should be put in charge of medicine for the whole United States at the very least. I think this book should be mandatory reading for doctors and medical school for all current primary care physicians, and maybe for our elected officials.
Dr Runge puts together six prescriptions in chapter 10 that need to be totally considered in the United States.

A very interesting book. I'm not a doctor and I don't work in healthcare, but I’m interested in AI and medicine and how exploring and investigating in that field can benefit people, dogs, and all. So, the book does cover complex topics, but is very easy to follow and well explained. I think the examples and stories do help a lot to understand for someone not working in healthcare.
I also like that the book touches all the benefits from it but also other sides of the issues, it is very balanced.
Overall, it is well written for both professionals and also curious casual readers.

I won this book on Goodreads and am glad I did. We are living in a broken healthcare system. Dr Marshall Rings has identified 9 great disruptors on the medical/healthcare horizon. The Great Healthcare Disruption is an insightful and timely read. I appreciated Dr. Runge’s balanced perspective. He doesn’t shy away from the hard truths about where healthcare is headed. Marschall Runge’s The Great Healthcare Disruption provides a compelling and highly optimistic look at the technological frontier of medicine, masterfully detailing how AI, gene therapies, and retail-driven care models could revolutionize the patient experience. I found his exploration of these "technological possibilities" to be genuinely fascinating, as he paints a future where precision medicine and Big Tech integration make high-quality care more accessible than ever before. However, the book largely glosses over the sobering reality that technological feasibility is only one piece of the puzzle; for true systemic change, an innovation must also be economically and politically viable. While Runge is an excellent guide through the laboratory and the clinic, he ultimately fails to provide a satisfying answer as to why the U.S.—despite possessing this world-class technology—continues to struggle with an abysmal healthcare system characterised by astronomical costs and uneven access. Without addressing the entrenched political interests and the fragmented economic incentives of the "medical-industrial complex," the book feels like a brilliant blueprint for a house that the US political climate constantly refuses to build.

- good points and perspective on industry changes
- interesting balanced approach to the issues related to healthcare as Trade offs and not “this is the right answer, there’s nothing wrong” (realistic)
Cons:
- not concise: this book could be cut down 50 pages. It’s almost like it’s built for skimming because there is so much fluff. The first 2 chapters were REALLY bad at this.
- not deep: the author has good perspectives, but doesn’t go deep enough to really explain things. It’s very surface level
I read books a lot of time to get depth. This is a weird balance where it doesn’t go deep enough to stick but is not shallow enough for sparknotes.
I don’t blame this on the author, he’s a CEO and dean. He doenst have time. This needed more editing. I get the need to put out quick because it’s a hot topic, but it could’ve used another pass
