In Denise River's Grumpy Reckless Doctor, a case of mistaken identity evolves into a deeply emotional journey of redemption and forgiveness.
Natasha has carried a torch for the brilliant Dr. Ethan Sullivan since medical school. When she lands a job at his hospital, she's captivated by his transformation from a once-clumsy acquaintance into an irresistibly seductive man. The chemistry between them is undeniable, culminating in a world-altering kiss. Natasha believes her heart has finally found its home, only to discover a devastating truth: the man she's falling for is not Ethan.
She has been deceived by Ian Sullivan, Ethan's identical twin brother, who has long lived in his more charismatic brother's shadow. Tired of being used as a stepping stone, Ian seizes the opportunity to be desired for himself. This deception, however, sets off a chain of heartbreaking consequences when the truth is revealed.
What follows is not just a story about the fallout of a lie, but a poignant exploration of its aftermath. River masterfully unpacks the emotional weight of betrayal and the difficult path toward healing. It’s a redemption story for Ian as he confronts his actions, and a story of profound growth for Natasha as she learns to understand the man behind the deception. Be prepared for an emotional read that will tug at your heartstrings; you'll want to have tissues ready.

Natasha has been in love with Ethan Sullivan since medical college but never had the courage to do anything. She takes advice from Rachel, her bodyguard and only friend, and when she finds him alone takes the chance and kisses him. She goes on to apply for a job at his hospital, but ends up working at his twin brother Ian's instead and though she's still keen on Ethan she starts to feel attracted to Ian as well.
Ian is the youngest of the Sullivan brothers and often mistaken for his twin. He's not as confident with women, who always seem to prefer Ethan to him, so when Natasha kisses him in the park he doesn't correct her calling him Ethan. When she then starts working at his hospital he's delighted and with Ethan away he takes on his twins' persona to let her down easily and win her over as himself.
A brilliant story full of lies, insecurities, steamy romance and plenty of humour. Told from the POVs of Natasha and all three Sullivan brothers (mostly Ian), it's lovely to see how all of their relationships are connected and how well they work as one big family (most of the time).
If you enjoy doctor romances I would strongly recommend all of the Sullivan brother's stories, as well as the others in this Small Town Romances collection.

When Natasha assumes that Ian is Ethan, Ian decides to impersonate his more charismatic brother twin brother, Ethan, to win her over. He has a long history of being "used" by women to get to his brother. He wants his moment to experience being the more desired brother. Not surprisingly, this deception sets off a chain of emotional consequences, particularly when Natasha discovers the truth. Natasha feels betrayed, and she walks away from what was growing between them. This book becomes a redemption story for Ian as he comes to terms with what he did and why. It's also a story of growth for Natasha as she learns that it's OK to accept what Ian did and why.
This is an emotional story that is difficult at times because you know that the fallout is going to be bad when it happens (and you know it will). There is a lot to unpack in this story, and it will take you to some emotional places. Have tissues ready.
Ian is the misunderstood twin. He is quiet and overthinks what he is about to say, and women only talk to him to get next to his twin. When Natasha kisses him thinking that he is Ethan, he uses it to his advantage and their path begins. When she learns the real truth will Natasha drop him. This small-town story is about a mistaken identity and how it turns into real love for two individuals who has had to transform their lives. It is well written and had me connecting with Ian and Natasha as they navigated their path to happiness.

-A mistaken kiss sparks a sweet and emotional small-town love story. I received a free copy of this book and am voluntarily sharing my honest thoughts.
Ian, the quiet twin, finally finds someone who sees him—even if it starts with confusion. Natasha and Ian’s journey blends humor, heartbreak, and genuine connection.
Tender, funny, and deeply felt—this is the standout of the Sullivan series. I especially enjoyed the sweet epilogue.
My favorite line “ I think I’m legally part of your wrist now”.
This is a fun, flirty, and emotionally charged read perfect for fans of small-town romance with a twist. The twist being mistaken identity - however how someone can not know that the man they are falling for isn’t who he claims to be is a little far-fetched.

She is about to find out he has tricked her by pretending to be his twin brother. Now he has won her over and he was her first and now she is expecting when she learns the truth. What will she do next? See just how it will all end I received an advance copy from hidden gems and an interesting read
Best book of the series! Wonderful writing and expression of how broken Ian was. At times you just want to cry for him. Loved all the Sullivan’s, but Ian is the favorite.
A case of mistaken identity leads to Ian Sullivan pretending to be his twin brother Ethan. Natasha thought she had always loved Ethan, but now she finds herself equally drawn to Ian and the version of Ethan that Ian is pretending to be. Will they work things out once the truth is revealed?
