From the author of the TikTok sensation Scars of Anatomy, this standalone is perfect for fans of hospital dramas and spicy medical romance such as Chicago Med and Scrubs.
"The romance is sweet, the banter is sassy, and the chemistry sizzles off the page." —Flynn Novak, author of Puck You
Delilah Harper is embarking on her biggest challenge yet after completing four years of medical school. With a residency at one of the best hospitals in the country, she’ll stop at nothing to achieve her dream of becoming a neurosurgeon. And that includes letting Bradly Gallow get the best of her.
Undeniably attractive, Bradly’s also arrogant and, unfortunately for Delilah, extremely intelligent. Since day one of medical school, Delilah and Bradly have engaged in a heated battle to mark their territory as the top student, acing every exam, excelling at every lab–lock step in terms of their achievements. As fate would have it, they match at the same hospital, carrying their rivalry across the country.
While battling to be the hospital’s top resident, the two begin the Brain Games—a competition in which they fight for the best cases and keep tabs to see who will finally get to assist during their first major surgery. The games seem simple enough, until a different sort of tension builds, and hidden feelings complicate their years-long rivalry. In this gripping and spicy medical drama, matters of the head and heart clash in ways that would bring even a neurosurgeon to their knees.
Brain Games was such a fun, tension-filled medical romance.
Two med students. One top neurosurgery residency. A rivalry that turns into a case-by-case tally game. And very questionable professionalism 👀
I loved this fun, high-achiever romance. The competitive energy between them? So good. The “I need to beat you, but I also want you” tension? Even better.
I loved the neurosurgery setting — it made everything feel intense and high stakes. Their case-tracking competition during residency was such a clever way to keep the rivalry alive even after they both “won.”
It’s ambitious, flirty, dramatic in the best way, and gives strong academic rivals energy.

This book is out now!
"And since when do you have a daddy kink?" "Apparently since you just said it."
This is my 2nd book by this author and I absolutely loved her debut so I was excited to be back in this world. This is an interconnected stand alone from Scars of Anatomy (5 star read) but you do not need to read the first one to pick this one up. We do get a couple of appearances from the couple Bronx and Olivia but the author did a good job with filling the reader in on who they are and their story. Which was nice since it has been a year since I read that one.
Our FMC Delilah is about to start neurosurgery residency at her top choice hospital Warner but she can't seem to escape her med school rival Bradly. The two of them have been competing and Brad wants it to end once and for all now that they moved up to being residents. But Delilah can't seem to let the competition go, enter Brain Games.
These two. I just love them so much! The banter and the chemistry between them was so strong! Delilah tried to deny it but it was so clear that Brad was into her. I've used this term before but he showed his interest quietly. He offered to take her home at the end of their day, bought her a new coffee machine (as a coffee addict SWOON), makes sure she does get home safe from the bar and just little things inbetween. Delilah was a little bit more stubborn on recognizing her feelings for Brad. If you are thinking you won't get any spice, think again! The spice was spicing and I just love how much Brad was into Delilah.
"Jack?" I call out timidly for good measure, making sure he's really gone. Silence. "Good. Now I don't want to hear another man's name roll off your tongue again."
There is a point in the book where you find it hard to root for Delilah. I try to be pro women but sometimes it is hard when you can see all the things happening around. She was not giving Brad a chance to explain his side and just automatically went to the worse things. Do I blame her? Not necessarily. And she does say that things just kept happening one right after another which made it harder. And yes I am purposely trying to be vague to not spoil it.
The last book was all in Bronx's point of view but this time it was all in Delilah's point of view. There were a few moments where I would have loved to have Brad's thoughts. Especially when things were going down with Dr. Larson (iykyk).
I do not know anything about neurosurgery to know what is accurate or not but I'd like to think that the author did research and took liberty with making it as realistic as possible. But if you work in the industry and are critical of that, perhaps proceed with caution just incase it is not accurate?
I absolutely love the friendship that Delilah had with Ashlee. Would love see her find her happily ever after!
There was just something about the storyline that sucked me in. I got to a point where I just kept thinking about them and wondering when they were going to discuss this feelings or when different things were happening. I was very happy that I had the holiday off to really binge a chunk of it!
If you are looking for a quick, spicy, workplace romance between two neurosurgeons - give this one a read!
Brain Games is such a fun and quick read, and perfectly scratched the itch that settles in my brain as I impatiently wait for every Thursday to watch the newest episode of The Pitt.
Delilah and Brad have been rivals since their first day of med school, and that rivalry continues as they begin their neurosurgery residency at one of the top hospitals in the country. With a truce called, though that truce becomes a tally game where the book gets its title from, their competitive energy and tension are higher than ever as drama begins to unfold.
I was not a medical drama person growing up; I didn’t tune into Grey’s, General Hospital, or Scrubs because they never called to me. When I say the neurosurgery department setting called to me, it called to me. I absolutely loved it, especially going in and having a little bit more of an idea as to what it looks like and how it operates — no pun intended. It creates so much tension between the characters, both in their personal and professional lives, that you can’t put the book down because of it.
Although there were moments where she truly got to me (I wanted to shake her shoulders and tell her to slow down on the conclusion jumping), I did love Delilah and Brad’s characters. Both of them grow and develop throughout the story, something I feel like I don’t always see when it comes to MMCs. She is independent and knows her worth. I adored the conversation with her mom towards the final chapters as she comes to see her flaws; it felt real and honest, even if it happens quickly. He’s just as sharp and protective, and I loved how he takes the initiative to talk to her when something is wrong, no matter how vulnerable it may turn out to be. All in all, it was refreshing to read and watch them reach their HEA.
I genuinely had such a fun time reading this book; my book slump is truly and honestly behind me now, thanks to Delilah and Brad’s story. Now, I have to politely ask and beg that Ashlee get a book because she’s for real my girl.

I loved this book! The rivals to lovers, banter and heartfelt moments were to die for!
I’m a new grad nurse who is obsessed with the Pitt and was raised on Grey’s Anatomy so this was right up my alley! Brad and Delilah were so cute ! The will they won’t they and the blatant he fell first and hella hard was fun to read!
I appreciated the honesty about hospital politics and how hard it is for women in healthcare especially women of color! I loved the representation!!
AND THE SPIIIIIICCCCEEE !!! Eye contact 👀, dominant, the words of affirmation!!!! Wheeeeeewwwwhhh !!!
Quick Summary: A young surgeons-in-love medical romance
My Review: Brain Games by Nicole Alfrine is a contemporary NA romance. It is being released today (6/16/26).
About the Book: "Delilah Harper is embarking on her biggest challenge yet after completing four years of medical school.
Bradly Gallow [will not] get the best of her.
...the two begin the Brain Games—a competition in which they fight for the best...
...the games seem simple enough, until a different sort of tension builds, and hidden feelings complicate their years-long rivalry."
In My Own Words: Long-time rivals are lovesick over each other, but secrets, stubbornness, and external pressures get in the way. Can they diagnose their problem before things go critical?
My Final Say: Brain Games was a mixed bag reading experience. I liked it, but... It was a bit up and down.
Initially, the pacing threw me off a bit. I got bogged down in jargon and elements that were a bit superfluous. Once it picked up (at approximately the 30% mark), however, it then hit a stride that kept me interested. The story moved through conflict points that were realistic and believable.
About the Characters: Full disclosure, I did not really like these characters. They were okay.
What I Liked (The Positives):
🩺 - I liked that the leads were aspiring neurosurgeons. 🌍 - I liked that Delilah and Brad's parentage was culturally diverse. 🧠🏆 - I liked the friendly competition angle. (I appreciated how it drove the two of them to excel academically and professionally.) 🤝 - I liked that they supported each other when it mattered most. ⚡ - I liked the way the high pressure residency situations were written, especially concerning the attitudes and expectations faced. (I thought the push-pull, fast-paced dynamics were communicated well.) 🩷 - I liked the bestie and her intended. 💪 - I liked seeing the emotional journey presented, regarding Delilah. As a woman of color in a high profile career position, I think the author did a great job of revealing the unique tension, challenges, and prejudices that she faced. 🎤💥 - I liked that the antagonist got what was coming to him.
What I Did Not Care For (The Not-So-Positives):
🚫 - Some of the character actions did not match the words that were confessed. 🫣 - The secret setup that later became an embarrassment was yuck. 🌡️ - The mercurial shifts displayed by one character toward another got old after a while. 🗣️ - Certain phrases were repeated a number of times. It did not land in the way that, perhaps, it should have. 📈📉 - Two of the support characters fell by the wayside and lacked consistency, as well as integrity. One showed up as nice, then she got stinky and pushy. The other showed up strong and then poof, he did a no show. It left me wondering why. (They both needed to get gone, but their behavior did not match up from one moment to the next, in a sense.)
In the end, B and D got their HEA. That was cool.
Other: Readers who enjoy enemies to lovers stories, rivals to romp situationships, and medical-based romances may like this story.
Special Note: This book will probably do very well as an audio listening experience. It will address the matter of pacing. It may also communicate a tonality that warms up the characters, which will make them easier to relate to.
Appreciation is extended to the author, to the publisher (Wattpad WEBTOON Book Group | W by Wattpad Books), and to NetGalley. Thank you for providing access to a digital ARC of this title in exchange for an honest critique. The words I have shared are my own thoughts and opinions.
i received a digital ARC of this book from NetGalley in exchange for my honest review.
This is an Academic Rivals, forced proximity, he falls first, medical romance.
This book follows Bradley and Delilah as they start their first year as Neurosurgeon Residency. They were rivals in medical school so when they both get into the same residency program, their rivalry continues.
On the first day of the residency, he wants a truce but she doesn’t trust him because of their past. So instead of a truce, they start a game called the Brain Game, where they track who wins each day. This leads to some pretty fun cases that they have to work on. They are working for one of the best neurosurgeons in the country and it becomes obvious that he is favoring Brad. This causes lot of tension between them.
All their time together causes huge spikes in sexual tension between the two. Their constant arguing and completions and now their forced proximity to each other had really fanned the flames of their attraction. When they finally get together, I was so excited because i thought they would be so good together and I was right. He was such a caretaker and he started that before they got together but after they got together it was such much more and so much sweeter.
I could see the third act conflict coming, even though I didn’t completely know what it was, it was expected but I really loved the resolution. I thought it was interesting and just a great grand gesture.
There were a few things I didn’t like as well. In the beginning when they are doing their 2 weeks of residency prep, he starts dating/sleeping with a nurse. Then later he says (and we could see) that he has been in love with her since medical school. I just get the ick when they get into a different relationship even thought they know they have feelings for the other MC. Just why? Also, I don’t know why he kept the secrets he did. The 2 things he kept a secret just didn’t make sense, I don’t know why they would be secrets. There were other things he could have had as secrets that would have been more understandable.
I did enjoy the writing and the basic story, even if some of the plot points could use some finessing to feel more natural.
4.5 stars! Okay, so I requested this one on NetGalley as soon as I saw the line “perfect for fans of hospital dramas and spicy medical romance”—because I’m a huge Grey’s Anatomy fan. I’ve watched every season, every episode, and I’ve never read a medical romance before, so I was instantly intrigued.
I knew I’d like it… but I didn’t expect to love it this much. And whew—this book is spicy. Definitely one of the more explicit romances I’ve read in a while, but honestly? Good for Delilah and Brad. Their rivalry and tension have been building for years, and the payoff was so worth it.
I’d give it 4.5 stars because I do wish they had communicated better. Brad should’ve been more open with Delilah, but she also could’ve talked to him instead of jumping to conclusions.
But! Since I can’t do half stars—and I loved everything else—this is a solid 5 star read for me.
Two brilliant neurosurgeons, rivals since med school, both landing spots in the same residency program (which is basically unheard of)… this was the perfect enemies-to-lovers setup.
I also really enjoyed Delilah’s character. Being a woman in a male-dominated field is tough, especially as a woman of color. While I can’t relate to every part of her experience, I definitely understand what it’s like to not be taken seriously or to have people look for a male superior even when you’re the one in charge. It’s exhausting, but she handles it with so much strength, and I loved seeing her stand up for herself.
Brad surprised me in the best way. I can totally see how Delilah misjudges him at first. He comes off as a charming, hot-shot guy who can talk his way into anything. But underneath that, he’s thoughtful, supportive, and far more sensitive than he lets on.
They both show real growth and eventually learn how to communicate and truly listen to each other—but getting there is what really challenges their relationship.

“Brain Games” by Nicole Alfrine ❤️🩺🏥 @nicolealfrine 📘 Format: Digital Reader Copy 📚 Pub Date: 06.16.26 ⭐️ Rating: ⭐️⭐️⭐️⭐️⭐️Stars
“Brain Games” by Nicole Alfrine is a refreshing book with romance, humour and emotional depth.
Delilah is an incredibly competitive Medical School student who has a longstanding academic rivalry with fellow Medical School Student, Brad. I can only imagine the cut throat nature of studying to become a Doctor, and this book features so much drama associated with that environment. I don’t want to give away any of the storyline, but I will say that there is much more to the plot than I was expecting and I loved how the story unfolded.
“Brain Games” was a more consequential story than I was expecting. I was expecting a typical light enemies-to-lovers rom-com and I got so much more than just that! This book was highly entertaining and I connected with the characters so well that I found myself surprisingly emotional at times during the book. I was really cheering for Delilah and Brad.
Be prepared that there was a lot of spice (though that is my least favourite part of the book), and if you are looking for that, you will find it here.
I recommend “Brain Games” for anyone interested in a great enemies-to-lovers romance with substance.
I hadn’t read “Scars of Anatomy” by Nicole Alfrine before “Brain Games”, but I have already requested it from Libby through my local library!

Delilah Harper is on the cusp of victory. After four very long, very hard years of medical school, the efforts of all that she's sacrificed are about to come to fruition on Match Day, where she'll find out the name and location she'll be spending the next decade-ish of her life truly becoming a neurosurgeon like she's always wanted. As a bonus, she'll also finally get away from her heated rival of the last four years, the hot-to-trot-and-unfortunately-academically-gifted Bradley Gallow - until he also manages to match at her #1 neurosurgery residency program, an unheard-of feat and a huge downer on what should be her day of triumph.
Determined to make the best of things - which includes working even harder than before so that she can get the attention (and hopefully the respect) of her neurosurgery attendings, Delilah is perturbed when Brad proposes a competition - dubbed "The Brain Games", Delilah and Brad will fight for the best cases and keep tabs to see who will finally get to assist during their first major surgery. But with the passion normally born from rivalry comes a different sort of passion, and the underlying heat and tension of the last four years may come to a head before either resident has the chance to say "time out".
If I've said it once, I've said it a thousand times - I love a medical fiction that is REALISTIC, and I'm about to give Nicole Alfrine an honorary degree because she clearly hit the books before writing a book about medical school and residency. This book captured the angst and jitters of Match Day and the anxious energy of graduation, moving, and starting fresh in a program on which all your hard work depends in a way that makes me wonder if the author herself is a doctor, and having just experienced my own Match Day and most of intern year, I felt like the experience was incredibly relatable and realistic to what a medical resident experiences (though I wish I was also at a program where I got so many weekends off!).
I especially love the attention to the realities of being a minority in medicine that were explored here, as well as the realities of privilege, class, nepotism, and the unseen factors that can make or break the career of the deserving - and undeserving - in medicine. It's a modern-day problem that, while better addressed than the past - still has a lot to answer for, and I hope the justice in fiction also finds justice in real life more often than not. On the brighter side, I love a medical heated rivalry, and making two brainiacs butt heads really worked for this haters-to-lovers romance - it even almost made me want to fall in love with neurosurgery (ALMOST). Congrats to the author on another delicious medical tale, and please let me know when the ED romance is coming, happy to be your consult call for that one!
Thank you to NetGalley and WATTPAD WEBTOOK Book Group for the opportunity to read this ARC!!
This was a 4 star read for me! As someone who REGULARLY consumes medical dramas on TV, this was a read that was right up my alley.
If you have ever watched Grey's Anatomy, the FMC, Delilah, gives Christina Yang, but if she was in love with neurosurgery and the MMC, Bradly, is the charisma of Alex Karev meets the brains of Derek Shepherd.
This enemies to lovers story gives us all of the perfect elements of a tension filled journey to love. Despite being notorious rivals, the forced proximity of being chosen to the same prestigious neurosurgery residence adds a level of competitiveness that I love seeing in an enemies to lovers romance.
I loved so much about this book is that we get to see them duke it out in a hospital setting, we get to find out that Bradly has been in love with Delilah since medical school, we get to see them grow as a couple, but all of this never takes away from the competitiveness of them being chosen to the same residency program. If anything, they were the definition of iron sharpening iron and I loved that most because in a lot of love stories, we get one half of the couple sacrificing their dreams or their intelligence to allow the other one to shine.
Overall, the tension was great and made for some good steamy reading. The hospital setting, love, and competition vibes felt like I was rewatching the earlier seasons of Grey's Anatomy and I loved that most because I have binged that show so many times. The spice was there, this book contains some of my favorite tropes, and this was such a great read that keeps you engaged from start to finish.
If I have to note any down sides, I would only have two. First, I would have loved for this book to be dual POV so that we could get Bradly's POV, especially when it comes to his reasoning for sleeping with the nurse, finding out that he has been in love with Delilah since medical school, and the some of the competition scenes. Second, there is miscommunication towards the end of the book and if it is one thing I can't stand in books, it's the miscommunication trope. It causes me so much angst lol
Again, this is a 4 star read that I highly recommend. Publication date is 6/16/2026 and I recommend getting your hands on this, especially if medical dramas are your thing!
There is something wildly addictive about a romance where two people are equally matched in ambition, stubbornness, and chemistry, and Brain Games delivers exactly that. Nicole Alfrine crafts a medical romance that feels fast-paced, competitive, and emotionally charged while still giving readers plenty of tension-filled moments to obsess over.
Delilah Harper is impossible not to root for. She’s brilliant, determined, and fiercely focused on becoming a neurosurgeon, even when the pressure of residency threatens to overwhelm her. Her dedication feels authentic, and the intensity of the medical setting raises the stakes in every scene. Residency is already brutal, but throwing her longtime academic rival into the mix? Absolute chaos in the best possible way.
Bradly Gallow is the kind of hero that initially makes you roll your eyes before completely winning you over. He’s cocky, frustratingly intelligent, and always seems one step ahead—which makes the rivalry between him and Delilah endlessly entertaining. Their banter is sharp, competitive, and full of tension that practically jumps off the page. You can feel years of history behind every interaction, and the slow shift from rivals to something more is incredibly satisfying.
What makes Brain Games stand out is how seamlessly the medical drama blends with the romance. The high-pressure hospital environment adds intensity without overshadowing the emotional connection developing between the characters. The “Brain Games” competition itself creates the perfect excuse for forced proximity, one-upmanship, and simmering attraction that neither wants to admit exists.
The chemistry here absolutely sizzles, but there’s also vulnerability beneath all the competitive posturing. Watching Delilah and Bradly slowly lower their defenses while navigating impossible workloads, professional ambition, and unexpected feelings made this romance feel more layered than your typical rivals-to-lovers story.
If you love medical dramas with strong personalities, competitive tension, workplace romance, and plenty of spice, Brain Games deserves a spot on your TBR. Think Chicago Med meets enemies-to-lovers with a whole lot of tension and heart.
I had the opportunity to read this book ahead of publication, and these are my honest thoughts.
Like in so many books, I much preferred the FMC of this one to the MMC. Dr Delilah and Dr Bradly have been fierce rivals ever since they met at medical school and when they surprisingly are accepted for the same very hard to get neuro residency. There is some competition to start with; the same dynamic they have had for years. Brad proposes starting a match called the Brain Games which unfortunately gets lost somewhere in the plot.
There not many things I loved about this book but I'll give credit where it's due. The writing style itself is very easy to read and immerse in. I'm fond of indulging in medical fiction, and there is plenty of pages set in the hospital which I liked. I wouldn't say it has Gray's Anatomy levels of drama, but good enough to be entertaining. There is mixed racial representation on accounts of both characters which I liked as a WOC myself, but I wish there was more discussion around it. Similarly, there is discussion of favoritism, misogyny, and sexism in the workplace and I'm sure it would have been more impactful had more time been given to its discussion.
Sadly, things that felt icky though, are endless. To start with, Delilah calls her parents Momma and Daddy. This just strikes me as so weird for a grown ass woman. I have suspicions it may have been for they sake of some jokes. My biggest critique of this book is the MMC. It would be understandable if he were crafted in a way that is supposed to read as a grey character, but I doubt that's the case here. So many of his actions and words felt painfully wrong, like he wasn't just trying too hard or "pursuing" her, but also kinda stalking her. For context, he very firmly keeps insisting that he drop her home quite a few times when she keeps insisting she doesn't want him to. There is miscommunication multiple times in this book, which I'm not a fan of.
And the final straw for me is the unnecessary amount of spice everywhere in this book when I couldn't even be on board with the MCs having any chemistry. I was genuinely wanting them to break up, because the pairing felt so odd to me. Needless to say, I wouldn't recommend this book unless you want just a ton of smutty scenes and to turn your brain off. I will still be looking into reading the author's debut and any future books since I think the writing has potential.
A medical romance set in a hospital environment, featuring a rivals-to-lovers dynamic between two medical residents. If you're a fan of medical dramas, I think this book will definitely scratch that itch. The setting is engaging, and while it could easily become overly technical, the medical terminology is either explained well or kept accessible enough that it never detracts from the story. The cast is diverse and layered, bringing both levity and emotional depth to the narrative. As this is a romance novel, be aware that there are plenty of spicy scenes throughout. I went into this book expecting a storytelling style similar to Nicole Alfrine's first novel, Scars of Anatomy. I was surprised to discover that this story is told entirely from the FMC's perspective, and if I'm being honest, she's not the most likable narrator. One of the things I loved most about Scars of Anatomy was its exclusive male POV, which made the story feel fresh, unique, and memorable. While it may be unfair to compare the two books directly, I found myself missing that distinctive element here. For me, Brain Games lost some of the uniqueness that made its predecessor stand out. As for the story itself, I found the beginning a little slow and not quite as strong of a hook as I had hoped. Part of that stemmed from the FMC's unwavering belief that Brad, our MMC, is her archnemesis and actively dislikes her. I found myself constantly frustrated at Delilah’s stubbornness and inability to listen. Even as their relationship develops and he repeatedly demonstrates that he genuinely cares about her well-being and success, she remains unwilling to reconsider her assumptions. While I generally enjoy rivals-to-lovers romances, I grew tired of this particular dynamic because the rivalry felt largely one-sided and relied heavily on miscommunication. Overall, the book was entertaining and easy to read, but I couldn't shake the feeling that something was missing. Thank you to NetGalley and Wattpad WEBTOON Book Group for providing me with an ARC in exchange for an honest review.
Okay but THIS had me hooked the moment I read the description. 👀🧠
I’m a sucker for enemies-to-lovers, a slightly possessive MMC (but not toxic pls), and anything science/medical related - so this felt like it was written for me. 🫶
At the start, I was a bit confused ngl. The first few chapters felt slower than needed, like we could’ve gotten that info in a much shorter way. I was honestly a little worried the whole book would feel like that… but thankfully it picked up pretty quickly. 🙌
And when the MMC suggested a truce early on?? I panicked 😭 I thought “no way we’re losing the enemies part already.” But THANKFULLY Delilah said no and the rivalry stayed alive. 🔥
The hospital setting + their “Brain Games” competition was actually so fun to follow. Watching them fight over cases, try to outdo each other, and just constantly compete?? Elite dynamic.
Now let’s talk romance. 👀 They definitely do not hold back lol… There are some detailed scenes, but a lot of them are shorter, which I personally didn’t mind.
What I really liked is that the story doesn’t just stop once they get together. We actually see them function as a couple - dealing with work, pressure, miscommunication (not overly childish 🙏), and real-life challenges. That always makes a romance feel more complete to me.
Bradly??? Sir??? Literally everything you’d want: ✔️ smart ✔️ confident ✔️ a little possessive ✔️ funny ✔️ secretly in love for YEARS
Like… who allowed this man to exist?
Delilah is also such a strong FMC - smart, hardworking, confident, and easy to root for.
I did predict a few things, but there were still moments that surprised me. 👀
My only wish? I would’ve LOVED Bradly’s POV. I feel like that would’ve taken this to another level.
Overall: a quick, bingeable read (I finished it in one day), with great tension, strong characters, and a fun medical setting.
Thank you to the publisher and NetGalley for providing me with an ARC in exchange for an honest review 💌

Thanks to NetGalley, Wattpad WEBTOON Book Group | W by Wattpad Books, and Nicole Alfrine for this ARC! I voluntarily give my honest review, and all opinions are my own.
What to expect! Enemies to lovers (FMC one sided) Games! He falls first AND harder Strong FMC in the workplace (relevant) Black cat FMC/Golden retriever MC (in my opinion!) Medical events!! (sorry I know nothing!)
I did not expect to absolutely love Bradley and Delilah as much as Bronx and Liv but holy wowww! I think Brad might be the first "book boyfriend" I actually adore as his own person AND with how he supports and loves Delilah. This is a stronggg (one sided) enemies to lovers with the FMC having nothing short of pure distain for her MMC. We start a secret game of the mind when both Delilah and Brad end up at the same prestigious hospital for their med school match and "compete" for the best cases.
One of my favorite tropes is he falls first AND faster and while there is definitely some spice between our MCs throughout the story, there is no doubt that Brad is absolutely enamored from day one while Del takes quite some time and serious self-reflection to see exactly how she feels. My least favorite part was the huge secret that Brad keeps for sooo long, but I do understand it was absolutely necessary, and we NEED the miscommunication and separation of these two for them to see things clearly on their own. One of the biggest things I loved about Delilah was watching her realize her own prejudice and negative mindset of her environment and the change in her feelings when she starts to become more open-minded. While some deserve it- I'm looking at you Jack - others like Dr. Allen, Ashlee and even Brad push her to see that not everything is black and white, and you don't have to go through fighting alone.
If you love growth and sass and just an overall amazing time I would 10/10 recommend Brain Games as your next read!!

4.5⭐️ and 2.5🌶️
This book made me feel like watching Grey's Anatomy and Scrubs and I really loved it! It was funny and cute and I loved the topes, rivals to lovers, he falls first (and harder) and a friends with benefits situation? Chefs kiss🤌🏼
The book itself is amazing, it talks about important topics too like sexism on the workplace (something especially prominent with doctors) and a little bit of racism. I teally liked how those topics were handled and I really liked how everything got resolved. The characters themselves I did love. Bradly was ADORABLE and omg I loved how he was always in love with her😭 I loved that trope so much! Delilah was a little but intense at times and really stubborn but her personality felt real and validated being a brown woman in a male dominated workplace.
There were two things I didn't like. The first thing was Delilah's attitude towards the end when she was being so stubborn that she wouldn't let Brad explain himself. I completely understand where she came from but it felt a tad over exaggerated imo. The secind thing was the Epilogue. And no, I'm not mad about the SPOILERS!!! LAST WARNING!‼️
The pregnancy. She specifically says that she wanted to be a mom someday so the pregnancy felt right. I'm talking about the lack of details in the epilogue. How many years pass since the ends of the book and the epilogue? It says they got married "a little over a year ago" and that she was 7 months pregnant. But how long has happened? She was still a resident so 4 years? 3? I really don't know and I feel like it could have been a easy fix just telling us the timeline. Other than that, I really liked the epilogue!
Again, thank you for the opportunity to read this ARC! All opinions are my own and I read this by choice✨
Plot and Vibe Summary Delilah and Bradly have been cutthroat academic rivals since day one of medical school. Now matching at the same prestigious residency program, they take their professional warfare to a new level by creating an intricate points system to win prime surgeries. The workspace boundaries completely shatter when their academic obsession morphs into a secret casual arrangement. This book serves a high energy mix of high stakes hospital adrenaline and intensely passionate workspace proximity. The environment feels like Grey's Anatomy combined with an ultra competitive academic workplace where neither side wants to blink first.
What I Liked The sharp banter kept the pacing brisk and engaging throughout the pages. The detailed neurosurgery settings felt authentically researched and added fantastic tension. Bradly is a beautifully developed male lead who falls incredibly hard while trying to maintain his competitive edge. The transition from strict rivals to casual partners felt earned and heavy with palpable tension.
What I Didn't Like The miscommunication tropes near the final act dragged out the resolution a bit too long. Delilah can occasionally feel stubborn to a fault which made certain arguments frustrating.
Final Thoughts Nicole Alfrine delivers a deeply addictive contemporary romance that balances brainpower with raw passion. It is a wonderful choice for readers who love smart characters earning their happily ever afters.
Tropes, Spice, and Content Warnings Tropes: Academic rivals to lovers, workplace romance, forced proximity, rivals with benefits, he falls first. Spice Level: 4 out of 5 flames with explicit descriptive encounters. Trigger Warnings: High stress medical environments, intense parental expectations, graphic surgical procedures, hospitalization of loved ones.

First off, thank you so much NetGalley and Wattpad WEBTOON Book Group for allowing me to review this ARC!
I was really excited about reading this book because it sounded right up my alley, but unfortunately it didn't work out for me as much as I had hoped that it would. There were many times during this book where I just found myself being uninterested, and it felt like everything was just repeating itself. The switch from being "rivals" to a spicy scene every. Single. Chapter. Was insane and felt like I was genuinely getting whiplash. More towards the end of the book, there was so much jealousy between the two main characters, Brad and Delilah, and it was annoying how he couldn't seem to let her do anything or fight any of her battles on her own. The miscommunication was insanely unnecessary and dragged out for no apparent reason. It did absolutely nothing for the plot.
Brad was honestly the best part of the book. In my opinion, the romance was never really there other than from Brad 99% of the time. His scenes really interested me and were honestly the reason that I continued with the book.
Delilah was one of the most insufferable FMC's I have ever read about. The miscommunication at the end was entirely on her, and there was, again, zero reason for it to even exist. Apart from that, her romantic feelings seemed to have sprung up out of nowhere, with no major realization or buildup. Everything with her and everything she did just felt copy-and-pasted, leaving me bored throughout the book.
This book honestly just didn't give me any excitement or want to keep reading. This could've been due to the fact that I didn't connect with any of the characters, but at the end of the day, it just fell flat.
Set in a fast-paced medical environment, this story is all about rivalry and ambition, which makes everything feel exciting and lively. The back-and-forth between the main characters is constantly entertaining, and honestly, some of the best banter I've read in a while. Their relationship has that enemies-to-lovers, enemies-with-benefits vibe, and it really works. The tension comes from their constant drive to outshine each other, but beneath all that, there's an undeniable chemistry that just keeps growing throughout the book.
The female protagonist is strong, sharp, and totally easy to root for. She knows her worth and isn’t afraid to stand her ground, which I really appreciated. The male lead has that confident, slightly cocky confidence that adds so much to their dynamic—especially as some deeper emotional layers start to surface.
One of the highlights of this book is the dialogue. The banter feels super natural, quick, and genuinely fun to read, making their interactions feel so real. The forced proximity adds even more fire to the mix, creating moments that are both tense and emotionally charged.
Sure, I would have loved a little more depth in some of the backstory and character growth, but honestly, it didn’t take away from how much I enjoyed the story. The pacing kept me hooked, and I kept reading just to see how their relationship would change.
All in all, Brain Games is a solid romantic read packed with tension, great chemistry, compelling characters, and that competitive spark that really makes it stand out. If you're into workplace romances, snappy banter, and slow-burn tension with a satisfying payoff, this is definitely a book you’ll want to check out.
Oh wow did I love this! Brain Games is incredibly Greys Anatomy coded, so if you like that show then you will 100% like this read. It combines a nice level of medicine with an rivals to lovers romance. The medicine part is actually well done and is focused on enough where it is believable and understandable but not the main focus of the story. The romance has so much chemistry and tension (also with a well done level of spice) and just incredibly well written overall. There are many bumps along the road, but nothing felt "too easy" and I really felt the stakes being weighed with every action. There is a point in the story where Delilah briefly sees two men and that didn't sit right with me, giving the plot lines but it was a short moment. The characters themselves feel very real. Delilah seems exactly like someone I'd meet in my undergrad and the way her and Brad treat their education is so real. It's nice to see academic characters that still balance their social life, as there are some really good side characters that I was glad got a good amount of screen time. I also really liked how there was a touch on the aspect of Delilah being not only a woman in the medical field but also a woman of color since she was half Indian. It really brought her challenges to light, which I didn't know would be challenges for her, so I enjoyed that eye-opening topic. I feel like my only wish was that we got to see more, I'd totally read another book following the Warner residents!! I really have nothing I didn't like, it's just a good read to pick up when you're bored! All in all, I really enjoyed this fast paced, medical drama/romance!!
As a nurse...I am a sucker for a medical drama. Brain Games by Nicole Alfrine did not disappoint. If Emergency Bar Services (EBS) I a real bar please tell me where because I definitely need to go there and hang out.
Delilah wanted to be a neurosurgeon basically her whole life after witnessing her cousin battle a brain tumor. She was bullied during her childhood years in school for wanting to learn and be the best. During her collegiate years she continued to feel pressure to be the best, constantly competing with fellow students to be number one in her class. Her top competition...Bradley Gallow.
Brad's parents are both doctors. He has worked hard during his collegiate years to compete with Delilah, also wanting to pursue a career in neurosurgery. Both anticipate going to two of the top programs for neurosurgery during Match Day when they are shocked to learn they are accepted to the same program!!
The first day, Brad tries to end the competition and call a truce but Delilah just can't trust that he is telling the truth. She is so conditioned to think she has to fight for everything that she can't see what is right in front of her. Her inability to trust and let her guard down almost ruins everything. There were so many times in this book I wanted to just scream at her to relax and trust those around her. Right when I am about to give up on her, Alfrine throws in the twist of all twists! I did not see that one coming. This was the perfect enemies to lovers/workplace romance.
Thank you, Webtoon Entertainment, for allowing me to read this arc in exchange for an honest review.
Hey, listen, I’m just a girl in her gap year, prepping and getting ready for MCAT life, and we all know I LOVE a medical romance book. Brain Games follows two medical interns, Delilah and Bradley, during their first year of neurosurgery residency. They’ve had a rivalry since medical school, which continues into the professional world. As their intern year begins, a truce is called, and the Brain Game (can’t you tell they love the neurostem?) is born. Reading this book was such a treat from a premedical perspective, especially since I’ve never had much exposure to neurosurgery beyond psych and my neurology course in undergrad.
Delilah and Bradley are incredibly well-crafted characters. Delilah is driven, fiercely ambitious, and shaped by years of competition, while Bradley’s dedication and skill make him a worthy rival. Their dynamic is intense, layered, and very real this isn’t your typical enemies-to-lovers story. Their interactions made me laugh, cheer, and yes, sometimes scream at them to just trust each other! Just when I thought I had their story figured out, the book threw in twists that completely surprised me.
Overall, reading Brain Games was such a fun experience. I fell in love with these characters and their journey, and it’s the kind of book that’s hard to put down. Perfect for anyone who loves a mix of medical life, workplace tension, and romance with heart.
Frankly, I couldn’t put this book down (which wasn’t difficult as I read it as a DRC). It was full of the cheesy, cliche rivalry that I was promised as soon as I read the books description. And I’m not upset. I actually loved the dynamic between the leads, and the friendships that laid everything out bare. And there was tension at practically every turn, except there lies the problem: this couple’s problems were solved too quickly. In a book that establishes that the two main characters as lacking words constantly, and saying so much while actually speaking few words, why were their conclusive conversations so very quick, providing every correct word learned through what should’ve been months of therapy? It might be controversial, but I would have rather have an inconclusive “I know we’re at rock bottom, and digging up, but there’s no one I’d rather dig with,” than, “I know exactly what’s wrong with the way I communicate, and you do as well, and we’re going to fix it.” But then again, this is a romance. They should be allowed to be happy, and there’s no need for realism. I would 100% recommend this to people who loved “You Between the Lines” by Katie Naymon… which I did. Maybe I’m just jaded that I liked this book so much for so many of the pages. This is definitely up there on my romance books I read because I was tired of literary fic and scientific writings. And, of course, this was my bread and butter as a Biology-Psychology Major Pre-Med student whose favorite discussion topic is the brain.
Brain Games was such a cute and entertaining read, and I ended up having a great time with Brad and Delilah’s story.
One of my favorite parts of this book was the medical setting. The hospital environment added so much energy to the story, and I loved watching the competition between these two play out against the backdrop of their residency. The rivals-to-lovers dynamic was so much fun, especially because neither of them was willing to back down when it came to proving themselves.
Brad and Delilah had fantastic chemistry, and the banter between them kept me hooked from beginning to end. What I loved most, though, was how obvious it felt that Brad’s feelings ran deeper than he wanted to admit. Watching that unfold alongside their rivalry made it incredibly easy to root for them.
This book also kept me engaged with a few twists and surprises along the way. I had several theories while reading that ended up being correct, but there was at least one reveal that completely caught me off guard.
As with any good workplace romance, there were a few characters who got under my skin, which only made me more invested in seeing how everything would play out. Between the close proximity, competitive tension, and emotional moments, this story struck a really enjoyable balance between romance and drama.
Overall, Brain Games is a fun, engaging hospital romance filled with banter, chemistry, and a rivals-to-lovers relationship that was impossible not to root for.
Thank you to NetGalley and Nicole Alfrine for the opportunity to read and review this book. All thoughts and opinions are my own.
Brain Games was such a fun and engaging medical romance that had me hooked from the beginning.
Delilah and Bradly have spent years competing with one another, constantly trying to prove who deserves to come out on top. Their rivalry stretches all the way from medical school into residency, creating plenty of tension and making every interaction between them entertaining.
One of my favorite aspects of this book was the banter. It was witty, competitive, and full of chemistry. Even when they were frustrating each other, it was obvious there was something more underneath all those years of trying to outdo one another.
I loved watching the transition from rivals to something deeper. It never felt rushed. Instead, their relationship developed naturally as they slowly began to see each other beyond the competition.
The medical setting added another layer to the story, and seeing them navigate residency while trying to achieve their dreams made the stakes feel even higher. Both Delilah and Bradly are ambitious, intelligent, and determined, which made their emotional growth just as satisfying as their romance.
Overall, Brain Games was a fun, chemistry-filled medical romance with great banter, plenty of tension, and an enemies-to-lovers dynamic that kept me invested from beginning to end.
A fun, fast-paced medical romance filled with competition, tension, and plenty of rivals-to-lovers energy. 🧠💕
Delilah and Brad have spent years pushing each other to be the best, from med school rivals to neurosurgery residents fighting for their place in a demanding field. Their constant need to outdo each other turns into a competitive little game that keeps the tension high, both inside and outside of the hospital.
I really enjoyed the neurosurgery setting and how it added extra pressure to their relationship. The high-stakes hospital environment, difficult cases, and ambition from both characters made the story feel exciting and gave their rivalry more depth. I love when both main characters are smart, determined, and passionate about what they do, and Delilah and Brad definitely delivered that.
Brad was such a fun character to get to know beyond his confident, playful side, and I enjoyed watching both him and Delilah grow throughout the story. Their banter and chemistry made this such an easy book to fly through. There were moments where the miscommunication had me wanting to yell at them a little, but I still found myself rooting for them.
Perfect for readers who love: 🩺 Medical romances 🧠 Academic/workplace rivals 🔥 Competitive banter 💕 Rivals-to-lovers 🏥 High-achieving main characters
Overall, Brain Games was a charming, dramatic, and entertaining romance that kept me turning the pages.
Thank you to NetGalley and the publisher for providing me with an eARC in exchange for an honest review.
Brain Games by Nicole Alfrine Rating: 4/5 stars High-Stakes Neurosurgery with a Side of Slow-Burn Romance Being in the medical field and loving dramas, I knew from the cover that I had to read Brain Games. The book follows first-year residents and competitors, Delilah and Bradly, as they strive to reach the top of their neurosurgery residency.
I loved the playful banter of the two main characters. Delilah is a strong female lead who refuses to be played down because of her sex or race. Bradly is a typical frat boy, attractive and flirtatious, never taking no for an answer. The story highlights the battles people of color and women face in being recognized for their achievements.
You can tell by the first few lines of dialogue between Delilah and Bradly that Delilah is the only one oblivious to the attraction they have for each other. The spice in the story is tasteful and well-placed, but it is not the main plot in the story. I personally loved that the book had more than just spice; it had a well-developed plot and backstory to support the relationship.
However, I felt the writing relied heavily on telling rather than showing. I would have loved to see more complexity in the writing, allowing for more subtext and interpretation. Overall, the book is very cute and spunky. This is a great choice for those who love medical dramas and romance, with the added inclusiveness of minority characters.
