Chart Like a Lawyer: Legal-Proof Documentation for Nurses & NPs
Chart Like a Legal-Proof Documentation for Nurses & NPs
by Jaime Weiland, MSN, AG-ACNP-BC, CLNC
Every nurse has heard "If it's not charted, it's not done." But few understand how those words can become the difference between protection and prosecution.
In Chart Like a Lawyer, legal nurse consultant and nurse practitioner Jaime Weiland pulls back the curtain on how your documentation can either defend or destroy you in a court of law. Combining her years of bedside experience with insider legal knowledge, Jaime teaches you how to think like an attorney when you chart - without losing your compassion or your sanity.
Through real-world examples, case breakdowns, and easy-to-apply charting strategies, you'll
What plaintiff attorneys actually look for in your notesHow to document critical thinking, not just task listsThe legally protective alternatives to vague phrases like "will continue to monitor"Common charting mistakes that place nurses and NPs at riskHow to use objective, defensible language that stands up under oathWhether you're a bedside nurse, nurse practitioner, or clinical leader, this book gives you the legal clarity and confidence you need to protect your license, your career, and your patients.
This isn't just about charting - it's about survival in modern healthcare.

Highly recommend this book to new nurses! The examples of what NOT to chart and what TO chart were excellent. The appendices at the back are helpful for referencing. However, I feel like some parts of the book were very repetitive. I definitely liked the explanation of depositions and different legal processes. I feel like I have a better understanding of the legal and medical perspectives.

Rather surface level and slightly misleading as its only RN applicable nothing covering NP scenarios. Good basic tips but left wanting more especially for NP side with covering differentials, ama discharges, capacity determination, how to document system delays like if imaging delayed or q consult is called but doesnt show for prolonged time etc.

I think every nurse should read this book! Especially as a new grad nurse trying to figure out my style of charting and what to chart/what not to chart, this book is so helpful and helps explains charting from a legal perspective. Will def be keeping this in my work bag to refer back to!! Highly recommend ⭐️⭐️⭐️⭐️⭐️

Very informative and eye opening. I also loved how it provided examples of what TO chart instead of saying what NOT to chart. Some parts were repetitive (I swear she copied a whole ass chapter and put it at the end), but overall I feel like it leveled me up when it comes to charting

Solid content, but for the price I was disappointed to see some errors in formatting/typos. It did also get a bit repetitive. Would be good for a new nurse.
