Good book.

The author explains the process really well and takes the mystery out of Root Cause Analysis. Definitely a book to buy!

This book was a quick read and spent much of it's content discussing how to conduct an RC investigation. The tools were briefly touched on and for the most part greatly ignored. 5 Why wasn't even mentioned,

There was some good things in there to think about when approaching RCA but I was hoping for more meat and potatoes and examples and less theory.
