Saw Dr Ludington-Hoe speak at a teeny-tiny midwives conference eons ago. KC has come a long way in acceptance by hospitals, but of course there's room for improvement. Can't remember much specifically about this book, the writing, the referencing, the organization. But it was among the first about an important topic.
