Despite the increasing visibility of trans and non-binary folx in media, political representation, and popular culture, their sexual lives and erotic embodiments are woefully under-attended-to in both scholarship and clinical practice. This book draws on Fielding’s experiences as both a trans client/patient and as a therapist to shift and expand the conversation and includes contributions from other trans and non-binary providers working at the intersection of gender-affirmative care and sexuality.
Trans Sex seeks to move trans sexualities from the margins of gender-affirmative clinical practice, to center pleasure, and to spark creativity and empathic attunement within the client-provider relationship. Whether they be mental health or medical providers, trainees, or seasoned practitioners in gender-affirmative work or sexualities, readers will be able harness creative strategies to enhance their practice and become more imaginative providers.
I came into this book curious about a range of concepts Fielding seemed to be offering, but was most pleasantly surprised at it's formatting as an accessible textbook. Written primarily but not exclusively for sex therapists, this book is laden with grey 'Theory into Practice' exercise boxes, alongside composite examples from her theraputic practice and life. While she by no means exhausts the extent of possible exploration of her concepts, these practical 'try this at home' interludes make it both tangible and practically useful. One of the most powerful parts of the book is their outline of 'gender-pleasure' as a propositional alternative to GAC/HRT's main priority being avoidance of gender dysphoria. I always aim to push my political thinking beyond 'what am I against' towards 'what do I want', so this intervention was welcome in a sphere where I hadn't considered how to phrase that kind of development.
Throughout the book, I noticed my resistances to many terms, phrases and manners of writing which Fielding uses. Many of these seemed to me on my initial read through, not bad, but cringe. It was interesting to reflect about whether breaking new ground in this kind of sphere necessarily involves this tone, until concepts and language has a chance to be sub-culturally filtered.
Trans Sex is a must read for queer people who go to therapy and the clients who treat them. This book invites you to rethink the ways you think about sex as exclusively performance focused with only one goal in mind (ie. orgasm). Fielding offers guidance on how to Bring Theory into Practice by giving actual tools therapists can use in sessions. Honestly... I plan to try some of these out with friends. We're lucky to have this book to turn to.
So many parts of this book I loved. From clinical applications, to challenging the language we use, to considering the ethics of curiosity. Lucy is a gem in our field!
I read this for a class as a counselor in a masters program and I really enjoyed this! The clinical implications were so useful and I really liked the combination of narrative storytelling and direct clinical work.
This book is such an authentic journey into the stories of eroticism and gender that is rarely heard. This author is a gem and the book is an important piece of work. Highly recommend!

The focus on concrete clinical practices, example vignettes, and conversations/research from other clinicians really helped me see the framework and take concrete interventions and strategies into my client work.
I took a lot of tangible resources from the book; in comparison to my more academic readings; this book was a wealth of information in a digestible size. Yet it still manages to feel comprehensive; serious shout out to the writer!
