The story is narrated in the voice of Kate Downey, an anesthesiologist working at a university hospital in Florida. Her husband Greg, a military doctor, has been in a coma for nearly a year after an explosion and consequent brain injury when he was deployed. Kate lives with her loveable and slightly eccentric great-aunt Irm and lab, Shadow. At work, she has been placed on probation by the Chief of Staff, Dr Walker, for an incident involving some wrong medication which impacted an organ donation case.
Attending the funeral of a friend of her aunt’s one day, and a little later the same day, that of Dr O’Donnell, former president of the University, Kate’s suspicions are roused for both were cases on which she had worked. Both patients had undergone minor procedures and were fine when discharged, yet both died within two days of their surgery. Finding that both had been operated on by Dr Charles Ricken, an arrogant and unpleasant surgeon who has nearly everyone’s hackles up, she decides to at least bring up the matter with him. But he reacts strongly and she finds herself at the receiving end of a reprimand from Dr Walker, who is already against her. Her Chair professor, James Worrell supports her but asks her to tread carefully. But she soon finds herself blamed for malpractice and the subject of an inquiry.
Meanwhile Kate runs into Dr O’Donnell’s son Christian, a lawyer, who tells her that he and his brother Luke (a doctor) are also looking into their father’s death. Christian and Kate, her bright intern Jenn and great-aunt Irm (also remotely, Luke), begin to look into the matter piecing together clues, and finding more such cases along the way.
Alongside, there is also further trouble at the hospital for Kate as a student who isn’t up to the mark with his work tries to pin the blame on her raising all sorts of false allegations; while at home, Greg’s brother Adam is fighting her to put an end to Greg’s life support, something Kate is unwilling to do, for it would involve literally starving him to death.
This was a thrilling and engrossing read which kept me hooked all through. From the point when Kate pretty much stumbles upon the two suspicious deaths, to piecing together the few clues she has, to being joined by Christian and the others in her investigation, discovering other cases and the whole journey of tracing down the killer to prevent him from causing any further harm, I enjoyed the mystery at every step. This is not so much a whodunit in that one has clues to the possible who/whos fairly early on, and the why begins to make sense too, some way into the story. But despite this, the author kept me reading to see how they would catch up with the killer and whether they’d be able to stop him in time; there are surprises and events that one didn’t expect all the way. Also interesting was watching Kate work out the ‘how’. Some of the scenes at the end though I found to be a touch too dramatic (though this might be just me because I often have this reaction in mystery thrillers).
Reading the story, one can tell that it is written by a person who has experienced the pressures of working in a hospital, and in situations the characters deal with, and dilemmas they must face. Even before I read the author’s note at the end confirming this, it was evident. She conveys well, how working in a hospital isn’t in some ways, very different from other human institutions for besides the pressures of work, and the split second decisions that doctors are so many times faced with, there is also the inevitable politics of things, and being on the wrong side of the powers that be is never a good place to be, as poor Kate soon realises.
The characters in the story I found to be really likeable—Kate herself is a character one really feels for (and roots for) and she displays great strength and tenacity considering all that she has been going through in her personal and professional life. Christian too, carries baggage, but proves to be a good friend and partner in the investigations (there is also a line of possible romance explored which was quite nicely and subtly done). Jenn is enthusiastic but also naïve, while Dr Worrell is a great person to have backing one. My favourite character though was probably great-aunt Irm, who not only always supports and has great meals (there’s plenty of food) for everyone, but who seems to be able to gauge people’s personalities, even when she has not met them. Aunt Irm is spot on every time about who can be trusted and who can’t. And of course, I also loved all the doggos in the story (4 of them)—but that goes without saying.
The issue that Euliano tries to discuss through the mystery is as I said a really relevant one—the prolonging of the life of those who are terminally ill or in a vegetative state, and relatedly the issue of living wills. This throws up so many questions in terms of who is or should be the right person to make that decision (particularly when one leaves no living will), and whether the kind of death a patient will undergo (not always peaceful) is justified by relieving the patient of longer term suffering? More so when one can’t be certain the person taking the decision has the patient’s best interests at heart.
This was a really great read, and even before I finished I was already wondering if the author is working on any other books, as I for one will be excited to read them!

Fatal Intent, a debut novel by Dr. Tammy Euliano is a story that is gonna make you think, and think hard. A debatable topic of ending the life of a non-responsive patient is described with such sympathy that as a reader there were parts of the story that made me cry buckets.
DR. Kate Downey, is surprised to know the deaths of her patients after they have been released from surgery. A simple query to the practicing surgeon opens up a can of worms and when approached by Christian, the son of the deceased she has no choice but to check the possibilities of something seriously gone wrong. But Kate is herself not in a good place as she fights for the survival of her husband who has been in a vegetative state and moved to fulltime care facility. As Kate battles her brother-in law in keeping Greg alive, professional turmoil in the form of a vengeful student adds to her woes. The going is not all bleak as Kate finds support from her aunt, Christian and Jenna, a 4th year med student.
As the author is herself a practicing anaesthesiologist, the reader is given glimpses of how cases get handled by the team and the administerial and official policies involved in a hospital, and in spite of all the checks how the greedy ones still conduct their evil acts. As thrillers go, medical thrillers have always been a favourite of mine but honestly it has been a while since I got my hands on one. Fatal Intent has some emotionally charged moments with some brilliant writing that blew me away.
The villains are revealed early in the story as the reader plunges into the investigation concerning the deaths along with Kate but it is the climax with a numbing scene of ‘save or kill’ that left me torn to shreds.
Awesome debut! Highly recommended.

Fatal Intent by Tammy Euliano is a fast-paced medical mystery. Because Dr. Euliano is a medical doctor, her knowledge and experience makes this story seem authentic and informative. Anesthesiologist Dr. Kate Downey becomes aware that a few patients who had a simple surgical procedure at her hospital died a few days after their operation. Because they had already been released from the hospital before their death, the hospital is not likely to be held responsible for the tragedies. But something is definitely not right and Kate’s superiors do not share in her enthusiasm in detecting the cause and manner of the unexpected deaths. Is there a common denominator? Did the problems occur on her watch? Could this be happening at other hospitals? Is this intentional and therefore a crime? There are many interesting characters, the good people and the not so good people and sometimes it is difficult to establish which is which. There is plenty here to keep the reader turning the pages. For anyone who has ever had surgery, this will leave a chill but Fatal Intent is well-worth the read. I remember the thrill of reading Robin Cook's Coma at its release in 70's. I was hooked and a medical thriller fan was born. “Fatal Intent” is a medical thriller that deals with the end-of-life issues, the DNR orders and the difficult decisions that love ones, attending physicians and nurses have to face. In fact “Playing God” is always difficult when one pulls the plug on a respirator and allows the end of life to occur. This is the fictional story of anesthesiologist Dr. Kate Downey who became aware that several patients were dying after what had been minor surgeries. Kate decided to look into the matter but some people in the hospital hierarchy did not support her investigation into the matter....
The narration slowly set the stage and picks up speed at the right moment to intrigue and engage us. Although I admit the huge cast of characters is a challenge to keep track of but that aside the drama is written in such a way that those not familiar with the medical jargon can easily follow the plot. For excitement, the story has a fair share of twists and turns to keep us going and guessing till the very end. This is a gripping read and an interesting subject of euthanasia or mercy killing and those who are willing to do the deed for a buck.
Kate is not alone in her mission to flush the person behind the incidents Christian O’Donnell, a lawyer and the son of one of the victim offers to help her find the truth and the culprit. Will they be able to get to the bottom...yes drip by drip well placed clues will give the duo the final solution....with a bang no less...The climax reveals the killer but what I did not see coming was by the manner in which it was done. Excellent.

A gripping fast-paced medical thriller. I was on the edge of my seat many times during the story. A number of patients at the hospital where Dr. Kate Downey is an anesthesiologist are dying within days of undergoing fairly minor surgeries and she, with help from a family member of one of the deceased persons, sets out to get to the bottom of the deaths. Kate is a very likable protagonist as are some of the other characters. The story touches on end-of-life issues and is very well written; you don't feel like you need a medical degree to understand it. There are many characters to keep track of, especially near the beginning, so you need to pay attention. This book was very easy to read and I'd definitely read another book by this author. Highly recommended.

Because the theme of this novel is so controversial, it would likely make a great choice for a book club discussion. If life is no longer worth living, if a particular loved one is a drain on resources, if there's no hope for anything other than a slow, lingering death -- who should decide when that life should end and how should it be done? It's a question that many readers may need to answer in their own personal lives, and it's a good one. The way the topic is handled, and the plot and action of this particular book, however, really left me reeling a bit. I had a very difficult time with the main character, her behavior, and the constant, convenient coincidences that allowed her to pursue the chase to solve these crimes. The romance aspect also really left me rolling my eyes. I usually love any type of novel that has a medical aspect due to my own background, but i just couldn't pull off the necessary suspension of disbelief as events spiraled to the predictable conclusion.

A quick paced medical thriller that places the blame for the rapidly growing deaths of elderly patients after they undergo minor surgery on the wrong person. The surgeon so nonchalant about the deaths is quick to write them off but Dr. Kate Downey is out to find the truth. With her career at stake she investigates the real reasons for the deaths. But this means putting her life on the line, threats and breaks ins. This is a gripping novel that you cannot put down as it keeps you in suspense and wanting Dr. Kate to make it out unscathed.
Great Book that makes you think about end of life issues
This was a great medical thriller. I enjoyed it in that respect too. But throughout the book, many end of life issues were brought up making you really think about about them and how you feel about them. For our kids' sake and for each other we will soon be preparing our living wills.

Dr. Kate Downey is an anesthesiologist who has a lot on her plate at the moment. She is trying to do what's best for her husband who is in a coma and in a vegetative state, on probation at work, and is trying to understand why some of her patients are dying mysteriously.
When she starts to notice a pattern in patients, she questions one of her colleagues. She doesn't want to risk her own probation status so does everything by the book when it comes to reporting these types of issues. But who can she truly trust? No one in the higher-ups of the hospital seems to be listening to her thoughts and opinions on these patient deaths. So, she takes matters into her own hands and decides to investigate on her own.
She becomes close to one of the victim's sons, Christian. He is a lawyer and helps whenever she has questions and she helps with some of his medical questions. They form a bond and friendship as they are both looking to find the correlation between all the deaths.
As they start getting closer to revealing who is behind everything, things start to become more dangerous. Kate wants to protect everyone close to her like her Great Aunt Irm, Christian, and one of her medical students. She feels the pressure to protect and save everyone and is forced to make some tough life or death decisions.
I give Fatal Intent 5 stars. I loved the medical aspect of the story because the author is a real anesthesiologist and readers will be able to feel the genuine nature of a medical setting. I also enjoyed the mystery of the story. Just when you think Kate and Christian are going to uncover the truth they are hit with another obstacle. I think readers will be shocked at the reveal of the mystery person causing all of this and all the secrets revealed.

This is a well-written medical drama. As the author, herself, is an anesthesiologist, I was expecting to see a great deal of credibility. I was not disappointed. The author has done a fine job dealing with thought provoking end-of-life issues. Plenty of twists and turns kept me glued to the pages It's fast-paced, suspenseful from start to finish.

Kate Downey is an anesthesiologist at a large teaching hospital in Florida. Her life is anything but smooth - her husband has been in a coma since an IED explosion and she is on probation - one more issue and she'll be fired. She becomes aware that several of her elderly patients have died after routine surgery. It isn't normal and she decides to investigate despite the warnings that she has been given. The surgeon who performed the surgeries isn't worried about it because according to him - old people die and it's no big deal. Her investigation into these deaths jeopardizes not only her career but her life and the lives of those she loves. When she is put on leave by the hospital, she works even harder to solve the mystery of the deaths and to clear her name.
This is a fast paced medical drama with an end that I didn't see coming. It puts the spotlight on end of life care and who should be making the decisions to let someone die. Once I started it, I couldn't put it down. Kate Downey was a well written and very likable character who wanted to protect everyone in her care as well as her friends and family. I hope the author writes more books about her in the future.
Oh my, this will raise some book club debate. I am sure. What a tremendous. impact story this is. When patients with underlining health problems have routine surgery at the hospital, they are dying. unexpectedly, within days of discharge. Dr Kate Downey, an anesthesiologist, has to put her career on the line to find out why so many patients are dying as she seems to be part of a lot of these surgeries. The more she pushes for answers, the more danger she is putting herself in. I enjoyed this medical thriller which made me think of the quality of life of a patient. Do we selfishly sometimes keep a loved one alive for our own need rather than for the good of them? It brings up my ethical thoughts and makes you stop and think. I liked Kate, who has a husband who was reliant on medical intervention to be kept alive. It lets you see both sides of the picture and opens your mind to what you think the right thing would be to do. It is so different looking into a situation rather than living it. The story builds and becomes tenser in the last few chapters. It is a book to make you think. I wish to thank Net Galley and the publisher for an e-copy of this book.
It’s been a bad week for Dr. Kate Downey, all of a sudden her patients are dying after simple surgical procedures. On probation for another incident, she is in the hot seat, especially when she starts a quest to find out just what is going on. Joined by her med student, Jenn, her Aunt Irm and Christian, the son of one of the deceased, she is soon deep into the mystery. It isn’t long until Kate finds herself on medical leave pending an investigation, and when it becomes personal, Kate fights back harder. A well written medical thriller by someone that knows what she’s talking about. This was a very fast read with characters that you can’t help but root for. I definitely wouldn’t mind catching up with Kate, Christian and Aunt Irm in future books to see where the next chapter takes them! Dr. Kate Downey is experiencing something most drs do not. The deaths of her patients. As she tries to find out more information her colleague tries to place the blame square on her. She must find out who or what is causing the deaths and quickly before her job is in jeopardy. This is a great psychological thriller that has everything needed to keep you entertained and engrossed from page one till the end. Speaking of the end, it will blow you away. I did not see that coming and that made the book even better. This one is worth a read to find out what happened.

Florida anesthesiologist Dr. Kate Downey has a lot on her mind. Not only is she grieving over the accident her beloved husband Greg suffered, leaving him in a vegetative state, but she also mourns a little girl she lost through miscarriage. Greg's brother Adam is stressing her out further. Adam wants to pull the plug on Greg, but she disagrees wholeheartedly.
In addition to these stressors, she learns that several of the patients she administered anesthesia to for minor elective surgery are dying at home a few days later. This new revelation proves to be disturbing, and Kate needs to know what is causing these patients to pass within days of their surgery—many of them are elderly.
Dr. Charles Ricken, a surgeon Kate distrusts and dislikes, claims she is at fault and places responsibility for these untimely deaths on her. To save her career and good name, she must prove her innocence. But where does she begin and who can she trust?
Jennifer Mason, a gifted med student Kate mentors, is also concerned. Could she have caused these fatalities? Kate confides in Jen about her worries as well as in her great aunt Irm, a feisty and sharp-as-a-tack senior citizen who lives with Kate.
Dr. Ricken claims Jen botched the intubation of the first patient who died, yet Kate insists that although there was some some difficulty at first, everything went smoothly, and no way could anything she did cause a fatality.
When Kate attends the funeral of Dr. O'Donnell, one of her first deceased patients, she meets his lawyer son, Christian. She praises him for his lovely eulogy of his father, yet she is somewhat worried he may blame her for his father's demise. When he invites her to grab a bite to eat, he mentions his brother wants copies of the records from their dad's surgery. This raises red flags for Kate. Could they really consider his death her fault?
When other patients die shortly after surgery—and those who were ones in Kate's care—distrust of her runs rampant in the hospital, and she is temporarily suspended from duty. Kate knows something sinister is going on, and to save her good name and career, she decides to investigate.
Kate and Jennifer peruse all the surgical records of those who died, trying to find a common factor. Kate asks to attain some diagnostic testing on lab work done on a few victims only to learn said patients have been cremated, so samples cannot be attained. What next? Putting their heads together, Irm offers suppositions pointing to Brad Jernigan, a recently employed substitute Certified Registered Nurse Anesthetist (CRNA). He just happened to attend to all of those who perished. A search for him finds he has mysteriously left the job and the hospital.
Tension escalates when Kate hears from her friend Randi, whose attorney husband confides in her brother-in-law, that Adam impersonated Greg by selling their mother's home, which Greg received in trust, yet due to his current condition, the estate reverts to Kate. Adam, a reformed drug and alcohol addict, cannot be trusted, but Kate has other important matters on her mind.
When Randi discusses this, she tells Kate her husband is worried that Adam may be impersonating Greg in other ways. They are similar in appearance, and Adam is known to be devious. Kate asks,
"'What ways?'
"'Your mortgage, credit cards, bank accounts, he could do a lot of damage.'"
Kate is further apprehensive about Adam and his shenanigans, yet all she can deal with is being suspected of killing her patients, so she puts this situation on the back burner. Now her main and only concern is to find out the truth about those who perished under her care.
Christian befriends Kate and includes himself in the search for answers. When her home is vandalized, he comes to her rescue. It appears someone broke through the sliding glass door, and Kate’s dog Shadow sustains several cuts in his paws. She believes this is a random break-in. That is until a note attached to Shadow’s collar reading “Back off” is discovered. Could this be Brad? Though she refuses, Christian insists Kate and her aunt stay at his place, specifying mostly to keep Irm safe.
More tragedy strikes when Jen goes missing. This is right after she contacts Kate with a cryptic text message saying, "He's here!" Kate knows she’s been doing some sleuthing on her own, so could she have found this man who they’ve discovered is going under many aliases? How much worse can things get? Is Jen in danger?
"Fatal Intent," a fast-faced read, is both terrifying and captivating, with many intriguing subplots. This debut novel written with accurate knowledge of the medical field is straightforward and easy to understand without the addition of too much medical jargon. What is hair-raising is the possibility that someone so deranged could perpetrate such transgressions though they allege these acts are executed with good intentions—even if they are devastating and illegal. A definite must read for those who enjoy medical thrillers.

This story begins with Kate discovering that 3 of the patients that she performed anesthesia on have died. Since she was placed on probation, she decided that she needed too look into the cases to find out what happened. She brought this to the attention of the surgeon to see if he had any ideas. He just said that they were all sick and people die. Because of this conversation, the surgeon filed a complaint with the chief of staff as well as the Medical Board. In her defense, she explained to the chief of staff and the people dying as well as an explanation for a complaint filed by a study that she tried to help with his problem solving an simulation of a person going into distress. The student needed to be able to perform in order to pass her course.
Kate was place on a leave of absence, so she continued investigating the deaths of patients in her hospital. She along with Chris, a son of one of the patients, in addition to one her students. In the course of the investigation, Kate had her house broken into as well as potentially being killed while vising one of the patients that was still alive. Also, the pursuit of other cases which were similar as well as 2 that happened just recently in Atlanta and which resulted in the death of Kate's student.
After the Atlanta incident, Kate was placed in another predicament when she received a call that her husband, who was in a coma, required emergency surgery. The facts all fell in place while at this location.
To find out the events that happened at the hospital her husband was in, whether Kate was vindicated, and who all of the parties involved with the killings were, then you must read this book. You won't be disappointed.
“Fatal Intent” is a thrilling medical mystery from start to finish. For someone like myself who is not in the healthcare field, the plot is very believable and not outlandish. The author is a doctor as well so it is likely fairly accurate, although I cannot speak to this with certainty. The novel also touches on a lot of controversial topics, such as the right-to-die debate, and is sure to inspire a lot of conversation at book club meetings. I also loved that the novel is not bogged down by overuse of medical jargon, an issue that plagues a lot of medical thrillers. This means that “Fatal Intent” can be enjoyable and readable by both newcomers and veterans to the genre.
Right away the plot draws you in. It is easy to relate and be empathetic to the main character’s position and predicaments. As her life gets more and more complicated, the protagonist emerges as a hero and the reader just really wants her to succeed and find happiness. There are some killer twists along the way, and the action moves quickly. I hope to see more awesome novels from this author soon.

This is a story about a anesthesiologist who discovers that two of her recent patients have died. When a third one passes, she begins to question how all these deaths could have happened. This story does bog in the beginning. There are so many characters introduced, so much hospital protocol, and policies that are told to the reader that I began to wonder if I should continue or just give it up. I am so glad I continued. At around chapter ten, everything starts to fall into place. The characters become deeper and interesting. The storyline really takes off, and it was difficult to put this book down. Very good writing, and a subject that has sparked some controversy regarding moral, ethical, and legal issues.
A very good read. It gripped me from the start. The topic is an interesting one. I for one would not want to live a half life and would hope for a mercy killing, so my family didn't have to willow in my slow passing. We often joke about it, but it is a very serious subject with very serious consequences, I would hope by then there will not be any legal issues. All in all a very thought provoking topic.

Fatal Intent came out of nowhere for me and sucked me in. A thrilling medical story that has it's fair share of twists and turns with action and friendships to keep things real.
I am really hoping that the author has more books with these characters up her sleeve.
Right from the very beginning, I was gripped by this controversial but enthralling tale by Tammy Euliano. Elderly patients are dying after undergoing minor surgery and anesthesiologist Dr Kate Downey decides to find out why this might be, though not everyone at the hospital in Florida where she works is happy.
This is a fast-paced medical drama with an end that I didn't see coming. The author's characterisation was excellent, especially of Kate whose husband Gregg was caught up in an explosion, resulting in him being in a coma. Kate was a very likeable character who wanted to protect everyone in her care along with her friends and family.
A thrilling medical story that has a number of twists and turns, together with plenty of action and a friendship. All in all, a great début.

Fatal Intent by Tammy Euliano is a fast-paced medical mystery thriller. The story follows the main character Dr. Kate Downey who starts looking into the unexpected deaths of elderly patients after minor surgeries, which shouldn’t have killed them. Kate is an anesthesiologist and was part of the team involved in the surgeries. While she suspects the surgeon Dr. Charles Ricken, she does not have any evidence to prove it. The Chief of Staff is quick to blame Dr. Kate Downey as she is already under probation for a previous error, she committed during a heart transplant. Kate now has to prove her innocence to not only save her job but her career as well. And she must do it while caring for her husband who’s in a vegetative state after an accident while on duty.
When Christian O’ Donnell, a lawyer and the son of one of the victims, offers to help her, she finds herself with an accomplice and a good friend. Kate wrestles with her growing feelings for Christian while she feels guilty for her feelings when her husband is in such a state.
The whole story takes place within a two-three weeks window and is paced very well. Clues that lead to the original killer are placed throughout the book, plus one to the author for managing to do this in as subtle a manner as possible. One of the most annoying things I find in thriller novels is when clues end up becoming spoilers. Tammy Euliana has managed to provide the readers with enough to keep it interesting, but at the same time keep us guessing. Dr. Kate Downey is a lovable character and as a reader, I felt an instant connection to her. She goes through a variety of internal emotional conflicts during the two weeks of traumatic events as the story unfolds. It culminates by leading her to an impossible decision where she must make a choice on who to save among all her loved ones. An impossible choice for any human let alone a Doctor who has taken an oath to protect all lives.
The Climax of the story reveals the killer and the figure behind the killer. While I was able to guess who it might be, I was pleasantly surprised by the manner in which it was revealed. It kept me on the edge of my seat and had me turning the pages without missing a beat. The only aspect that I did not like was the stereotyping of POC. The author describes a Black Man for the security guard role, an Asian Ph.D. researcher, and a Latina Housekeeper. I would’ve preferred it if the author hadn’t done that and left their ethnicity out of the descriptions for these very minor roles that are a blink and you miss roles in the book. Religion and the right to live or die is a big concept of this book. However, the author has managed to base the mystery on this concept without making it controversial.
Overall, I enjoyed reading the book and recommend it to everyone who loves a good mystery thriller, especially those who love medical mysteries.
I give it a solid 4/5 stars.
