Drugs and surgical scrubs
Regulus Black has a successful career as a junior anaesthetist at a busy London hospital, but his brother Sirius has turned to drugs. One morning, Regulus comes home from a night shift and finds Sirius in his flat. He knows straight away that this could only mean trouble.
“Absolutely no,” Regulus shook his head. “No. Simple as. No.”
“Reg, please. There’s nowhere else he can go.”

Medical AU fic set in modern London, where Regulus is a doctor and Marauders are drug addicts. Rated mature for drugs use mentions and mild gore descriptions (nothing too graphic). Later chapters will include smut as well (M rating appropriate).
Eventual James/Regulus, where it is James that needs saving.
