This book addresses top-down institutionalization of usability (i.e., there is an executive or VP who is pushing it). It gives a good plan and explains common pitfalls. Because the book is written from a consultant's point of view, it can speak from experience having seen this process work (and not work) many times. On the downside, the reading can be dry and it definitely biases towards bringing in a consultant.
