Oh, you're going just by the blurb? Maybe it wasn't clear she was an active coast guard member when she got involved with an elisted and is now an active reservist as well as the ME for NCIS. So sorry for any confusion!
The Coast Guard isn’t like the other Armed Forces. They have no officers who are also doctors. All doctors, even flight surgeons, are not military even though they are still ruled by COMDTINST and the USCMJ. All the Coast Guard doctors come from Commissioned Corps of the U.S. Public Health Service, or USPHS Commissioned Corps.
Though I have heard that the Coast Guard just decided to send its own people to Uniformed Services University of the Health Sciences (USU), to become doctors.
But the key part, only she would have been courtmartialed. And that, most likely, would only have occurred after a formal meeting with her superiors where she would have been ordered to end the relationship. The Coast Guard typically does not go straight to the extreme. Now, had she been a Naval officer on a ship or a Marine officer on a ship it would have been different.
Sorry I don’t mean to be a pain. I really mean this with the best intentions. Their ranks were clear. I got that. I am a military brat, Marines Navy, & Army. I grew up in Southern California next to the USCG station at Dana Point Harbor. Also I am a professional researcher. I want the authors I love to have good info.
