Well, it really came from a couple of different places. It started off as just a page I had written for practice in creating vivid imagery. Then I had a dream. Then I had an idea I wanted to do for a character. When I was going through some of my old writings, I saw how the dream, the character, and the old practice writing could come together into what I thought would be a cool idea. As I worked through putting it all together, I was able to work through developing the plot points.

Dreams, conversations with people, and a vivid imagination : )

Well, I'm currently working on revising the draft for "Retribution", the sequel to Esther. I'm also outlining a mystery story, a steampunk horror novel, going over some ideas for a sequel to "In Search of Paradise", and working on the last, yet unnamed book in the Esther series.

Just write, revise, and write some more. Don't give up. Like anything good, it takes practice. A musician doesn't just wake up with a desire to perform a beautiful piece of music and automatically is good at it. Just because you have a story to tell doesn't mean you're going to be good at it right away. It takes a lot of practice. I'm practicing every day (sorry if you've noticed my 'flat notes' in my own writing), and I plan to continue practicing. It's not always easy. In fact, it's seldom easy : )

Getting ideas and thoughts into a story, and sharing with others those thoughts and ideas. Being able to go back and read it and think, yes, finally, the story I wanted to tell is out there, available for everyone wants to read it.

It depends because there are different reasons for writer's block, but most of the time, I just sit down, read what I've written (sometimes from the very beginning), and then start writing again, even if I'm not really into it. I find that as I put myself in place of my character and see things from their point of view, it quickly overcomes any blocks.
Sometimes plot points will give me a block, and that's a little harder, and I may have to let things percolate through my mind for a day or two, but again, it comes down to just sitting down and writing, even if I have to scrap most of what I just wrote afterward.
