Wow, that's a tough one. In a sense all books create a fictional world. I created one for the first story in my series, A Fortune to Win, which was a mythical but very pleasant island in the Arabian Sea.

I'm not sure...just that a Victorian era Highland Vampires book seemed to be the natural progression of the series, which began with two books set in the mid-eighteenth century followed by a Regency era romp.

Do't give up your day job. Oddly, I was more productive when writing was sort of like a treat to be waited for, struggled for, wedged in between my work and my home life.

Poorly. I've struggled with it for about ten years. The only solution I have found is to write, whether or not the output is any good and whether or not it's related to a current WIP. Journaling is okay, writing about a random topic that strikes your interest is okay too--just as long as you're writing.
