There is no one way that works, everything in trial and error. What I try is to lie on my bed and think about the story, letting what I know dance around my brain while trying to answer questions about where to go next, how to do it, which characters to use. Alternatively, I think about these questions in the shower, while cooking, while doing laundry. Mundane activities seem to work best when hashing the current problem out in my head. Once I have answers to some of the questions, I start to write them down and it goes from there.
