I like to do brainstorming sessions once in a while and this book started with an idea I came up with during that time. It has taken about three or four starts to get it where it is now, though.

I do not wait for inspiration. I set a deadline, how many words I suspect my novel to be, and then come up with a daily word count that I have to meet every day. Once I start on a book, I don't take any breaks. Not even for holidays or falling ill.

Whenever I suffer from writer's block, I resort to doing timed sprints where I set the timer for 30 minutes and just write what comes into my head. My hands aren't allowed to leave the keyboard. That usually gives my brain a good jumpstart.
