I tend to have scenes appear in my head like a movie that's playing. I then try to fit them into the story as I write it. Sometimes its scenes from a totally different story and I'll have to scribble down the idea quickly so I don't forget it. Normally its the characters who keep reappearing with new ideas for their stories that have their story written first because they are just so demanding.
