I usually sit down with a chapter (or short story) idea then just start writing the first thought that hits my head. It doesn't matter what it is - I can fix it later. At that point, I'm writing. I then try to keep my editor self away from my writer self, so I'm free to get words on the page. That's easier said than done but so important.
