I'll sit and mull over a story line for years before I start writing usually. Ideas will come from various places. But generally I find there are starting points for great stories everywhere. Small conversations can sometimes lead to big ideas for a story.

I just start writing what ever junk comes into my head and get the fingers flowing. It's an old trick I learned from another writer years ago. But the reality iis I never really get writers block. Story block is another matter... and a good walk often helps me to clear my head and think through the path of the plot.
