At the risk of sounding flippant, that hasn't been a problem for me. Before I start writing I know where I want my story to begin and to end. The biggest problem I've had is not being satisfied with my original idea. In my first novel "Mimi" I literally re-wrote several scenes six or seven times before I felt they were logical.

Study the basics first. Learn how to self edit. Find your unique voice. Also, honestly question why you want to write. Is it for vanity, commercial success or for some other reason? If your goal to try to make any best seller list ( now defined in many venues as anything in the top 23,000 titles in sales) find a literary agent. You have a much better chance at finding a decent publishing house. If you self publish, be prepared to work on promotion as hard as you worked on your project. Line up everything well before your release date. Have web sites, release parties,blogs, bloggers, media, local book stores, all set up ahead of time.

I have one novel, "The Devil's Sous Chef" finished and a biography that is almost finished. I'm simultaneously working on two cookbooks, a collection of short stories and I'm about to begin research for a murder mystery. After those projects I have two more works that I'm outlining.

The inspiration comes from reading great writers and great stories. Listening to great music is also inspiring- but I'm a terrible musician.

The idea came from reading about an actress' tragic life story. Then I fused her story with themes from my own experiences and with something I call tangential experiences-i.e. where my life would have gone if I had made different decisions over the years.
