Nhạc sĩ, tên thật là Phạm Duy Cẩn, xuất thân từ một gia đình văn nghiệp. Cha là Phạm Duy Tốn thường được xem như nhà văn xã hội đầu tiên của nền Văn học Mới hồi đầu thế kỷ 20. Anh là Phạm Duy Khiêm, giáo sư thạc sĩ, cựu Đại sứ Việt Nam Cộng hòa tại Pháp, văn sĩ Pháp văn, tác giả những cuốn Légendes des terres sereines, Nam et Sylvie, De Hanoi à Lacourtine...

Sinh ngày 5 tháng 10 năm 1921 (5 tháng 9 năm Tân Dậu) tại phố Hàng Cót, Hà Nội. Phạm Duy học vỡ lòng tại trường Hàng Thùng, học tiểu học tại trường Hàng Vôi, học rất dốt và thường bị phạt. Đến khi 13 tuổi (1934) vào được lớp nhất, ông học giỏi lên dần, thành một trong những học sinh ưu tú của lớp, nhất là môn đọc thơ tiếng Pháp.
