Well, it depends on how severe the illness is and how controlling the hospital is. Some facilities actively prevent people from making recoveries because the environment is stressful enough to make the delusions a necessary coping mechanism. It's also important to note that Thomas was sick, even before going through trauma. He could just be irreversibly insane.
