book editions need combined but prior publisher used my nickname ... > Likes and Comments

Hi! My book, Ink Vine, was originally supposed to release with Psychotoxin Press. The publisher made it live on Amazon long enough to order author copies for me, then removed it (I was not notified of this happening); when he did this, he used "Eliza Broadbent" instead of "Elizabeth Broadbent."

Ink Vine has now found a home with Undertaker Books, and it's finally live in preorder. But I'm having trouble combining the editions for obvious reasons—they don't have the same author name. The cover is the same; the blurbs are the same. Is there a way to resolve this at all? I'm devastated this happened in general and hoping for help.
