message 1: by Chris (new)

Chris Clevenger Thank you for such a thoughtful review. You captured one of the core ideas behind Operational Integrity in Action: recurring problems are often symptoms of deeper conditions within the operating system.

The 3P Diagnostic System was built around the interaction between People, Process, and Product because those areas rarely fail independently. When the same safety concern, quality issue, delay, rework loop, or productivity problem keeps returning, the most useful question is not simply who made the mistake. It is what condition inside the system allowed that outcome to repeat.

I also appreciate your point about the plant-turnaround storyline. I wanted the concepts of continuous improvement, accountability, operational excellence, and structural correction to be experienced in context rather than presented only as theory.

Your closing observation especially reflects the message I hoped readers would take away: lasting improvement comes from building a stronger system, not from asking good people to compensate for a weak one.

Thank you again for reading and for sharing your perspective.

Have a Safe, Positive, and Productive day!
