Safety, quality, accountability, process discipline, communication, morale, and productivity are constantly influencing one another. Lasting improvement requires leaders to understand those relationships rather than simply adding pressure or correcting the latest symptom.
I also appreciate your point about broader relevance. While the book is grounded in manufacturing, the underlying principle applies anywhere recurring problems become accepted as normal. When the same failure continues to return, it is worth asking what condition inside the system is making that outcome possible.
That shift—from managing recurring failure to understanding and correcting the structure behind it—is at the heart of the 3P Diagnostic System and the Operational Integrity approach.
