I think I'll try the exercise before writing tip :-]
I outline by chapter also, it's a good rule of thumb, you can even use Ingermansonn's Snowflake Method as a starting point and flesh it out to a chapter-by-chapter outline. I do all that in Excel, by the time I write word one it just about flows out of the outline.
...And STILL, there are changes that happen midstream that cause me to go back and change previous chapters.
