Is the cover going to be printed? If not then medium should be fine, otherwise go for large.

If you're printing it as a regular book, best to go with the largest size available, so you'll have as high a quality image as possible. The image can always be made to fit into a smaller size in whatever cover making program is used.You may want to contact Jason Christopher jsnchr@gmail.com for help with your cover.
I just had someone do a cover for me and I must say they did a wonderful job and I'm so ecstatic about it. They asked me about the size and dimensions of the book which I didn't know but not only did I learn how to finds this out but also found out an estimate of how many pages my book will be. I myself am doing my book myself on CS so I need to know these things before going ahead and getting it done.

When transferring that resolution to print you need a much higher resolution because it's actual, physical printed dots that make up the image.
In print the standard resolution has to be 300 dpi or else it looks fuzzy and the edges aren't sharp and clear
So from your original question small and medium are ok for covers that will only be seen online but if your end product is for actual printed books you would have to go with large and higher
