I dont think it was a secret so much as it was a traumatic event that changed her. major spoilers ahead. be warned.

I think the second one is the "secret" but really it was more like something that happened that Ella didn't want to deal with

The bridge thing I'm pretty sure only Ella and Micha knew about.
I think she left because she felt guilty about her mom and she didn't want to deal with it. She felt like if she was there she would be reminded of her mom all the time. Micha also served as a reminder but she also didn't want to deal with the thing that happened on the bridge. She was basically overwhelmed by everything and dealt with it by running away from her problems.
I wondered the same thing after reading this book, the title was catchy (and it seems to be the author's self-identifier...The Secret of...The Forever of...The Coincidence of...The Redemption of...) BUT it didn't jump out as obvious either. After thinking on it I decided that The Secret of Ella and Micha was that she loved him, because the entire book seemed to be the initial denial and then slow revelation for Ella that Micha was more than a friend. When she finally tells Micha she loves him, he says "I know, you told me that night on the bridge" and we learned that the night on the bridge didn't just end where Ella remembers it ending, but there was more that happened after they left the bridge, so Ella is essentially being let in on the secret her mind kept from her. Dunno, but that's how I worked it out for myself!

I guess I was thinking about the secret that Ella was keeping as opposed to the secret that the title was referring to.
The secret could be the truth about their relationship. That it means more that what both of them initially thought it did.
I just finished it last night . . . I also think the title means that it is "their" secret. that they are just "made for one another" "drawn to each other" each . . .

I think the secret is that they love each other. I mean, if you've been best friends with someone your whole life it may be awkward to finally admit to yourself that you are in love with this person. You risk losing a friendship that you cant live without if things don't work out.
In my opinion the 'secret' is what happened on the bridge. He found her right before she tried to kill herself, so I think that the secret is that she tried to kill herself on that bridge when he found her. I don't think that anyone else knew that she tried to take her life that night.
