If, due to character development, the wish desired for a quest should change, but has already been set, how changeable is it?

It is changeable as long as the character still hasn't entered the Labyrinth. Or he has to state his new wish to the Keepers the first time he meets one inside the Labyrinth. The wish is also debatable at the end, but the Keepers have freedom to grant the actual wish or only the one stated at the beginning of the Quest.
