House (what House was given points or had points subtracted; ex. Ravenclaw)

Points Added/Subtracted (how many points did the aforementioned character give to or take from the House listed above; ex. +10; -20)

Reason (why were points added or subtracted; ex. Jane Smith casted a jinx on John Doe; John Doe gave the correct answer in class.)
