What movie rendition of a book you have read altered the most details from the original story?

I have watched many a movie based off of a book that I have read, but I don't think any I have ever seen changed the original story quite like the movie adaptation of Blood and Chocolate by Annette Curtis Klause.
The book was a decent book and the movie was not bad but, to be honest, had the movie not had the same name or kept the names of the characters the same, I can say that I truly don't believe that I would have known they were meant to be one and the same.

Ella Enchanted I'd say, the movie has very few in common with the book.
The Lawnmower Man if we include short stories, the movie only took King's story title in order to be able to print his name on the poster.
