What's the Name of That Book??? discussion

This is a teen novel with a teen boy and girl who are linked together through their minds. The boy is in a wealthy family far from the girl and they have to hide their connection from the rest of the world because no one believes them. When they pass out or sleep, they "visit" the other.
I read this about six years ago. Can't remember the title or the author or any of the characters name.

If you can name this book I would be very appreciative!
