Lizzie, when did you read this? Was it hardcover or paperback? New at the time you read it or older? Would you consider it a picture book (like for pre-readers), or aimed at older kids but just done in pictures? Do you recall how long the book was, or whether it was oversize (like a picture book) or regular book size?

The format of the book was optical illusion/ispy/surreal st..."I would have read it late 90s/early thousands and if I recall correctly it was relatively new. It was definitely a picture book aimed at older kids. I believe it was roughly A4 sized.

Don't give up, Lizzie -- sometimes it can take several months or even a year or two. Feel free to come back and "bump" the thread once a month (to bump, just add a comment). Were the illustrations in color or black and white? If they were in black and white, maybe Phobia by John Vassos. The cover doesn't show any of the illustrations, but you can see some here.
Rolf Heimann's Phobiamania in 3D maybe? There are some inside pictures on ebay: http://www.ebay.com.au/itm/Rolf-Heima...

I remember one page having a girl climbing a tree and climbing out on a branch to try and get her cat, and the branch was very thin. A few pages later the tree branch was snapped and her shoe was on the ground, and she had a broken leg. Her fear was heights of course.
Another I remember is two children at a lake taking photos, and the Loch Ness monster is there, getting in their photos, but they don't notice. A common theme in the book is cats, umbrellas, mushrooms, and taps. Please help!
