The Sleepwalker's Guide to Dancing question

I am wondering about the meaning of the grandmother's ghost in the picture Amina took for a school assignment. Also how come Amina sometimes saw Akhil as well when she didn't have a tumor - though I guess it could be a manifestation of grief. Any ideas?
