A woman physically attacks a man, and he grabs her arms to fend her off, leaving bruises. She hits him with a book and breaks his nose. He swats her away and leaves a bruise on her face. Should he be arrested for abuse?

No idea. Whether he will might depend on the state. Some states have laws that in any domestic altercation that gets in any way physical, both parties are arrested and it's sorted out at the police station.

That seems fair to me, as long as the authorities recognize that abuse goes both ways. I don't like it that a woman can injure a man and then call foul when he defends himself. Equal rights; equal responsibilities.
