The Sex Gates Portals appear at random all over the world. If they don't make you vanish when you pass through them, they cause you to emerge as the opposite sex

Girl Bradley Barker goes into hospital to have his wisdom teeth removed. The very day he enters the hospital, the porters receive their redundancy notices and decide to get drunk. The result is that he is wheeled down to the operating theatre and has a sex-change operation by mistake.

Middlesex Callie/Cal is intersex raised as female though genetically male. After her/his condition is discover Callie's parents and the doctor want her have sex reassignment surgery HE does not want the surgery done.

Peter Meilinger | 469 comments I heartily second the rec for A Civil Campaign. Bujold is very good at exploring how technological advances will change things, and relatively easy complete sex change on a world where only men are allowed to hold power is an interesting mix. It’s just a fantastic book in its own right, too.

For other recs, two superhero books come to mind.

Dreadnought by April Daniels is about a trans girl who gains superpowers that transform her into her perfect female form. This causes problems with her family and friends. Haven’t read it but I’ve heard nothing but good things.

Princess Holy Aura by Ryk Spoor is about a middle aged man who acquires the powers of a Japanese-style Magic Girl, including a transformation into the form of a teenaged girl. Haven’t read it yet but I’ve enjoyed everything else the author has written.

And if you haven’t seen the new Jumanji movie, I recommend it. It’s a great adventure yarn, for one thing, but it also has a female character trapped in a male body. It’s mostly played for laughs, but Jack Black actually did a really good job with it.

These all sound great, Peter thank you. I haven't seen the movie yet the trailer looks good. Maybe there will be a novelization of Jumanji.

Keith | 224 comments Iain Banks' Culture books depict an advanced, utopian society where a sex change is relatively easy (people have a lot of control over their own biology in general, e.g. they can produce safe drugs inside their own bodies). One of the protagonists of Matter is a woman from a more primitive civilization who was brought to the Culture when she was young, became a man for a time, then went back to being a woman.

In Marco Polo Sings a Solo a young man deposits sperm in a sperm bank, then gets a sex change. She is then impregnated by his (now her) sperm and gives birth to a child. She is the mother and father of the baby.

rue | 42 comments I generally tend to go for the temporary disguise gender-bender type of book, so I'm not sure if these fit your criteria.
All Men of Genius and The Girl Who Was A Gentleman are both historical fictions, set in England, and All men of Genius is one of my personal favorites. It's kinda steampunk if you're into that.
Babe in Boyland and Chaotic Good are modern day. Not really much to say because I don't usually enjoy realistic fiction as much as a lot of other genres, but you may enjoy them.

Keith | 224 comments I remembered another one - Unmasking Miss Appleby, a Regency romance with fantastical elements where the heroine obtains a gift of shapeshifting magic from a fairy indebted to her family. She uses it to turn herself into a man in order to get a job as a secretary to an earl, then (of course) falls in love with him and uses her powers to help him deal with his enemies. (It contains explicit sex scenes, if you'd rather avoid that.)
