What's the Name of That Book??? discussion

Looking for a book where it starts out in college. The hero and heroine are best friends but he has a girlfriend. He’s leaving for medical school I think and she gets up the nerve to tell him how she feels at a bar I think. He rejects her kind of and leaves her out on the patio and She ends up getting raped by the guys brother. Fast forward 5 years later and the guy is back in town and had no clue what happened but wants to reconnect with her. She has the kid which was the result of the rape. Does anyone know what book I’m talkign about?!
