What's the Name of That Book??? discussion

Possibly early 2000s YA Novel (I remember reading it in 2007 but it was an older book). The setting was an island and the girl runs off to be with a guy on the island that her dad did not approve of. The boy later gets sick or injured and ends up losing his leg.
Island/ small tribe / hunter gatherer society / they lived in a hut and ate mangos that I remember and I felt that the cover of the book was red.

Allison, is this a historical story with survival elements (e.g., Paradise by Joan Elizabeth Goodman)?
