I didn’t know what was going on but thought it was related to drugs. Later I thought she was going through withdrawal.
I didn't make the connection, I just thought they were sedating her because of her injury, but some of that pain being from withdrawal makes sense
To tell the truth I thought it was someone dying a horrible death only to finally been giving something to ease the pain so the terrible psycho could start all over again with torture later. And then once the story started I forgot about that prologue until just now. Seriously, I just now remembered there was that weird prologue that I had no context for. Wow. I'm special.

I thought along similar lines, that this was the killer at work. It was an odd way to start the book, with a perspective outside of either Sarah or Frank. It's not repeated again in the series.
Me either. When I read this book for the first time I recall checking to make sure I had picked up the right book because it was so out of the norm for the series up to that point.
It was bizarre and never really connected later. We’re here still conjecturing. Terrible device.
I'm glad to see I wasn't the only one thrown by the prologue. It was odd and I don't feel as bad now that I put the whole thing out of my mind.
