What's the Name of That Book??? discussion

Hi!
I am looking for a romance book. It's very, very angsty.
The h comes back to her hometown to make amends to the H. I think the H lost someone, may be a pregnant wife because of the h. Maybe it was an accident or the h did some mistake. He is a doctor and works at a hospital though he is still very depressed. The H hates the h very much. She goes to him and tells him that she wants to repay her dept so he can do whatever he wants with her. At last the H says ok and makes her his ''mistress''. She is more like a body he has sex with whenever he wants than a real woman to him. I think the h even moves in with him. Everytime he sleeps with her it's angsty. One day or night, I think she gets enough of the H's cold, cruel behavior and they fight. The H rapes her and she leaves him. The h turns out pregnant. The rest I can't remember. The romance was very bittersweet and I just loved that feeling. I read it 4-6 years ago and was too stupid to save it. Usually, I don't like to read about doctors.

Maybe Unforgiven by Elizabeth Finn I haven't read it yet, but the plot reminds me of the reviews I read of this book. And I think the H is a doctor who hates the h but I don't know why!

Yes, it's the one. THANK YOU SO, SO MUCH. In my head it had a little different plot.
