I interpreted the knocking figure as Tyler. Steve has been driven mad by the realization that humanity is this monster and his role in unleashing this monster - hence the sewing of his own eyes. I also interpret this as Steve becoming the witch by the same dark power that originally imprisoned/empowered Kathryn.

I’ve been wondering this ever since I finished it a few weeks ago so I had to just ask! :D
Also, I wanted to give it 5 stars but I can’t rate it until I understand what the hell.
