Augmenting this, Mrs. XX is acceptable when this is her married title. In the above example, Mrs. O.F. Walton is correct, because she was not called Octavius Frank Walton, but used Mrs Octavius Frank Walton as her title. If we removed the title, we'd be erroneously calling her "Octavius Frank Walton" which is obviously incorrect :)
I personally treat these names as being akin to pseudonyms, like "Pope Francis".
