There are visiting hours for people to see them who aren't in the hospital, and they can visit other patients' rooms with supervision of a nurse.

They can wear whatever they have given to them, but the insane asylum offers plain clothes, much like the ones you see patients wearing at regular hospitals, you know? :)
Is anime allowed to be used for an appearance?
Sure! I find it no problem when somebody uses anime. Animation helps stretch beyond the limits of what your character can look like. I get really irked when people restrict it. So yes, as long as you include a description of the appearance, as well.
