I think he knew that Millie was someone special and he just couldn't bring himself to kill her. He related to her protection and love for her son as something that was rare and special and something his own mother did for him.
I agree with Anita. He compared Millie's situation to that of his mother. He wasn't going to let her be killed on his watch.
He knew he was obsessed but the depths of feeling were still anathema to him. For someone so closed off, it would take time for him to admit his feelings as the rest of the book reveals.
I don’t think he understood what he was truly feeling, just responded to his impulses. He was just as confused about it as everyone else except for those closest to him. They knew. And, they knew it was all foreign to him.
